Job summary

Do you want to work in a dynamic mental health team? We are looking for an experienced Band 6 Social Worker Practitioner to work in The Havering Home Treatment Team, providing flexible rapid response and assessment to people in a mental health crisis 24 hours a day. The team is a partnership between the London Borough of Havering and NELFT.

Candidates should possess excellent communication and interpersonal skills, and an ability to work under pressure. You will be required to have an understanding of both acute in-patient and community-based mental health work, and the needs of mental health service users from a diverse range of backgrounds. You will need to be an excellent communicator, creative and capable of thinking on your feet. You should have a valid driving licence and have access to the use of a car for work. All successful candidates will be offered a full training package before starting in post and access to ongoing training, reflective practice and supervision. The Home Treatment Team is a 24 hour a day service so staff will generally be expected to work a flexible shift pattern.

Main duties of the job

Be a qualified social worker with a relevant qualification (DipSW or Degree) and have experience of working in mental health

o have experience of working with service users experiencing severe mental health difficulties and with complex needs

o have experience of managing risk in partnership with service user and carers.

o possess excellent assessment skills; be able to determine appropriate evidence based interventions and proposed outcomes of intervention for Service Users and their Carers

o have a sound knowledge of relevant legislation, policy and practice issues

o be committed to a multi- disciplinary way of working within a dynamic team, which promotes innovative practice

o be confident in managing complex cases that reflect the diverse needs of people with severe mental health problems.

About us

Probationary Period

This post will be subject to a probationary period. Internal applicants are exempt from the probationary period (unless you are an internal applicant currently part way through a probationary period or currently a bank member of staff).

Starting with NELFT

NELFT place a great deal of importance on new starters being properly welcomed and inducted into the Trust. All new starters will join the Trust on the first Monday of each month and will undertake a comprehensive induction of up to two weeks which will include mandatory training, systems training and the allocation of equipment. As part of the process new starters will have the opportunity to also meet the executive team, senior managers and attend a number of drop in sessions focussing in engagement, health and wellbeing and key processes. The induction will be held at our head office in Rainham, Essex.

High Cost Area Supplement - Outer London

This post also attracts payment for High Cost Area Supplement of 15% of the basic salary (with a minimum of £4,551 to a maximum of £5,735 pro rata for part time).

Ensure Information and Advice, Assessments, support planning and reviews are undertaken within the principles of the Care Act 2014.

Provide appropriate early Interventions to prevent long term needs and to promote independence and choice.

Ensure assessments facilitate choice and promote independence by applying the principles of Personalisation.

Support people and carers to ensure assessments, support plans and reviews are outcome focused and asset based.

You will be expected to train as a BIA and/or an AMPH.

Make autonomous decisions using the legal framework, in assessing under the Care Act and Mental Capacity Act.

Ensure people who lack capacity receive appropriate support and if required advocacy to express their views and wishes at any stage of the assessment, support planning or reviewing process.

Produce high level written and verbal evidence-based reports which are concise, informative and appropriate for organisational requirements.

Monitor progress of work, provision from other agencies and changes linked to each service user to ensure service users and carers needs have been met.

To seek advice and consult with senior staff in relation to, funding authorisations, specialist equipment and major adaptations.

Ensure service users and carers are fully aware and understand the Councils financial assessment and personal contribution process for charging.
