Main duties of the job

We are looking for a Band 6 Team Leader to join to join our Urgent Care Service based in Maidstone. 

Our Urgent Care Service provides a two-hour response to patients in crisis at home. Our aim is to prevent unnecessary hospital admission and facilitate early discharge from the acute hospitals. We also provide unscheduled care out-of-hours for patients who may be at the end of life, or experiencing an urgent need for healthcare.

We carry out detailed holistic assessments, developing personalised plans of care in collaboration with patients and their carers. Our patients are complex, and require proactive risk management, in order to be maintained safely at home. We maintain clinical oversight, and ensure all actions are completed appropriately.

Additional Working Pattern: 30 hours a week including long days, night shifts and twilight shifts

Qualifactions

Essential criteria

Registered health care professional (NMC registration ) / minimum of 3 years of experience in health improvement environment (delete as appropriate) Health related degree and evidence of relevant post graduate development at Level 6 Evidence of Specialist qualifications relevant to area of practice ( examples –:non-medical prescribing; mentorship , coaching , clinical expertise)-: Qualifications to be added by service

Desirable criteria

PRINCE 2 or equiv project management training PTLLS/DTLLS, PGCE or Certificate in Education Management or supervisory qualification or experience of study Working towards Master level study

Experience

Essential criteria

Minimum of 2 years’ experience in area of practice Practical experience in managing health related projects/complex caseloads Supporting others (mentor / educator / supervisor ) experience
