Job summary

The Integrated Transfer of Care Hub are looking torecruit Discharge Co-ordinators (previous applicants need not to apply) to work on the wards across the acute hospitalto increase patient flow and optimise patient experience surrounding alltransfers of care.

We are looking for energetic and caring people who have anunderstanding and some knowledge of the health and social care environment tocoordinate discharges and work within the multi-disciplinary team.

This is a seven day service that provides single point ofcontact across all wards and hospital sites within Mid Yorkshire Hospitals Trust for staff, patients, families, carers and associated peopleand co-ordinate / contribute to the safe and timely hospital discharge inpartnership with other multi-disciplinary colleagues.

Main duties of the job

To promote the correct implementation of dischargeprocesses throughout Mid Yorkshire hospitals to ensure safe and timelydischarge and smooth transfer of patients.

Actively participate in the ward based dischargeactivity ( SAFER, EDDs, board rounds and the identification of patientsfor transfers to PMU and DDH) to ensure thatdischarge activity is occurring for every patient within the ward. To maintain momentum of discharge planningthroughout the entire process.

Provide concise and detailedinformation/handover regarding patients discharge plans.

Work closely with all internal and externalpartners and agencies to ensure a seamless service, including working to reducedelayed discharges.

To link closely with clinical Site managers toensure effective use of beds.

To promote best practice in achieving highquality discharge planning

To be able to use problem solving skills, todetermine appropriate actions and formulate specific and appropriate managementplans.

To promote best practice in achieving highquality discharge planning To use tact and diplomacy where there is disputeover complex issues regarding leaving hospital and there may be disputesregarding courses of action to be taken. To liaise with relevant staff and agencies on adaily basis to resolve daily operational issues and ensure discharges aremanaged effectively. To support the Discharge Sisters and hospitalstaff in managing difficult discharges. To understand and apply the skills and knowledgearound complex needs and discharge planning, within a multidisciplinary contextin order to effectively self-manage a complex, multiple caseload. By applyingknowledge of skill deficits, components of self-care, productivity and leisureto ensure that comprehensive holistic assessments have been carried out toenable a smooth and effective discharge. To apply knowledge of the effect of disabilityand provide advice on discharge planning processes, options/ services availableto facilitate discharge and the roles and requirements of the multidisciplinaryteams. To use knowledge, training and experience toanalyse, plan, and implement discharge plans for patients with complexphysical, mental health and social needs. Using knowledge and understanding to supportstaff from across agencies in planning effective discharges. To be able to use problem solving skills, todetermine appropriate actions and formulate specific and appropriate managementplans. To have a working knowledge of currentlegislation, National Service Frameworks, clinical standards and nationalpolicies in order to provide an effective discharge service.

Communication

To maintain and promote confidentiality in linewith national and Trust standards and policies. To facilitate, establish and maintain clear andeffective communication networks with a variety of people, professional andexternal agencies. patients, carers, multi-disciplinary team, externalagencies and services etc. To communicate complex patient relatedinformation to facilitate collaborative working with other colleagues acrosshealth, social care and housing to ensure the delivery of a co-ordinatedservice. To effectively manage and prioritize workload,seek support and escalate if work cannot be completed in a timely manner. To adapt approach when information may besensitive, confidential, distressing, where conflict of opinions may occur. To inform colleagues, patients and relatives ondischarge planning processes, services available and how to access them. To promote an understanding of the role of thedischarge team and the discharge planning process within the hospitals and thewider community. To maintain productive working relationshipswithin the Discharge team and ward teams using clear communication strategies. Participate in departmental andmultidisciplinary/multi agency meetings as delegated by the Ward Manager, DischargeManager or Discharge co coordinators.

Administration/ Documentation

To be responsible for the written recording ofup to date, accurate information in line with Trust guidelines. To abide by legal requirements governingconfidentiality and data protection. To be responsible for the completion andcollation of appropriate data. To act on written information and referralsreceived from other members of the multidisciplinary team and outside agencies.
