The post holder will work independently within Urology cancer services, using their own initiative to support the specialist Urology nursing teams within Cwm Taf Morgannwg University Health Board (CTMUHB).

A key element of the role will be ensuring co-ordination of care for Urology cancer patients across the cancer pathway from diagnosis through to end of life.

As a member of the specialist Urology team, the post holder will provide support and care to the holistic needs of adult cancer patients, ensuring that cancer patients have support in navigating through the often complex health and social care systems.

Main duties of the job

The post holder, as part of the specialist Urology team, will :

Ensure consistency, quality and safety is at the heart of specialist Urological cancer nursing across CTM sites. Support the named key worker to work in partnership with the person affected by Urological cancer and their family, implementing holistic cancer care through co-production. Provide a high standard when implementing clinical care, and give comprehensive service co-ordination support for the specialist cancer team. Be responsible for the communicating condition related information regarding any changes noted in patients' condition to the appropriate key worker or member of the Urology team. Maintain accurate and contemporaneous records of each intervention, ensuring patient records are updated as necessary. Co-ordinate cancer nursing quality service assurance (audits/patient satisfaction) and establish/enable participation for Urological cancer nursing teams to ensure service outcomes are demonstrated. Play an active part in supporting the implementation of change management strategies as a response to new findings or direction. Facilitate and support the development of a cross-UHB approach to specialist Urological cancer service delivery.

The post holder will plan, organise and prioritise own workload as part of the specialist Urology cancer team, ensuring a person centred care approach is adopted across teams.

May be required to re-prioritise and adjust workload as the need arises.

Will liaise with the specialist cancer nurses regarding deadlines and priorities for referrals, follow up calls, administration support through development of framework and standard operating procedures for the Urology cancer nursing service.

Must effectively liaise and communicate with specialist cancer nursing teams, lead clinicians and other non-clinical professionals within cancer services and with other external bodies (NHS Trusts, Health Boards, primary, secondary, third sector and independent sector settings and South Wales Cancer Network) in the functional support of specialist Urological cancer nursing care co-ordination using a variety of communication methods e-mail, telephone, Video Conferencing etc.

Deal professionally, promptly and sensitively with escalated problems, issues and complaints.

Responsible for communicating complex condition related, sensitive information to specialist Urology cancer nurses, lead clinicians, clinicians and other non-clinical professionals and external organisations/care sectors regarding patient records, diagnoses, appointments and interventions adhering to UHB policies and procedures relating to sharing information.

Show tact and diplomacy when requesting information verbally, electronically or in writing from other departments or hospitals.

Have a working knowledge of cancer services and associated pathways.

Possess a level of expertise in the use of computer technology to enable accurate data collection, analysis and the provision of reports and other documentation linked to the role such as records of meetings, presentations, service activity.

Knowledge of a full range of nursing procedures acquired through Vocational Level 4 or similar level of qualification, or Vocational Level 3 plus additional relevant specialist training in a range of nursing procedures and practices.

Knowledge of cancer health services in Wales.

Occasionally participate in surveys, audits and quality assurance procedures as necessary.

Possess varied experience of planning and supporting implementation of service improvements across nursing or allied health professional services.

Have a knowledge and understanding of key NHS Wales strategic objectives such as the Welsh Governments Together for Health and its associated Cancer Delivery Plan.

Be required to have standard keyboard computer skills to fulfil duties of the post, and to ensure accurate input and retrieval of data and information.

Support the specialist Urological cancer nursing team in the collection, analysis, and interpretation of a range of service associated evaluation outcomes. This will require the skills for deciding how best to present data once collected and analysed.

Project manage and deliver complex service improvement projects involving setting up and populating of a range of databases to analyse and compare services across South Wales.

Develop and compare a range of options based upon complex facts or situations.

Be responsible for supporting the specialist cancer nurses in identifying and recommending innovative solutions to workforce associated problems identified where there may be a range of options available.

Will have regular contact with patients and carer representatives both in clinical settings and via other methods of communication, providing condition related advice and information concerning appointments.

Adhere to Cwm Taf Morgannwg University Health Board (CTMUHB) policies and procedures, and may comment on proposed changes to policies affecting cancer services.

Be responsible for the safe use of clinical and IT equipment, and will also be required to order and monitor stock and supplies used by the cancer nursing teams.

The post holder will be responsible for the day to day supervision of support staff and demonstrate own duties to new or less experienced staff.

Person Specification

Qualifications/knowledge

Essential

Educated to Level 4 Credit Qualification For Wales (CQFW), Vocational Level 4 or possess equivalent skills, experience and/or knowledge.

Desirable

Knowledge of the patient cancer pathway Understanding of the importance of high quality information and associated confidentiality issues.

Experience

Essential

Proven experience of, and ability to work well with, a range of cancer associated stakeholders including clinicians and managers. Experience of Multi-Disciplinary Team working. Experience of working in cancer services.

Desirable

Experience of working with Canisc or cancer patient administration systems.
