Navigation Hub Referral Management Assistant

The post holder will work as an essential member of the team in the Navigation Hub, working independently and managing own workload.
The post holder will be assisting the clinical and social care staff in the Navigation Hub to ensure the safe and effective management of all electronic supported discharge referrals coming into the hub. This will involve liaising with a wide variety of stakeholders and the post holder will be based in Royal Glamorgan Hospital (interim).
The role will include reviewing incoming electronic referrals, and using IT processes to assist with triage/escalation, and processing of referrals to the point of patient transfer. This may involve the co-ordination/booking of Non-Emergency Patient Transport, along with liaising with external providers such as Community Hospitals, Care Homes, Community Care Teams, Local Authority Teams, Community Health Care (CHC) Teams and the Voluntary Care Sector.
The post holder will liaise with numerous members of staff and wards to ensure there is effective, efficient communication and management of discharge referrals. They will also gather important data from the Navigation Hub for the production of reports, to enable the Navigation Hub team to make decisions on patient transfer into the most appropriate available community capacity, whilst minimising any discharge delays. The referral routes will be defined by appropriate protocols and triggers.
We’re looking for a motivated and conscientious person to work in this newly-formed team delivering Discharge to Recover than Assess model. You will be working alongside a number of specialist staff from varied clinical and non-clinical backgrounds.
This post is fixed term / secondment for 3 months to meet the needs of the service.
The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply.

**The post holder will**:
- Required to manage a range of highly complex and sensitive data or information and will need to display a very high degree of confidentiality when processing this.
- Collect, collate and report a range of supported discharge referral information and report to the designated Discharge Liaison Practitioner and Social Worker for any advice and guidance, operating within Navigation Hub Guidelines and protocols.
- Identify and record received discharge referrals for a range of supported discharge pathways on a daily basis, liaising with all relevant staff to plan the allocation of appropriate community capacity to these patients.
- Liaise with staff on multiple wards/units to ensure timely referral management, as well as timely responses where there have been requests for additional information, to prevent any unnecessary/avoidable discharge delays.
- Develop and operate a clear process that will ensure the list of referrals held in the Navigation Hub of those who have become unwell (ie: not clinically optimised for discharge) up to 72 hrs of original receipt, whereupon the referral will be closed.
- Sensitively communicate this to the referring ward.
- Support ward staff with data gathering relating to the patient discharge referral relating to the e-Whiteboards and WPAS.
- Support Local Authority colleagues with populating relevant information from eToC referral forms onto WCCIS and creating appropriate records where necessary.
- Produce regular reports and updates for the Navigation Hub manager on referral activity and flow into, through and out of supported discharge community pathways.
- Regularly liaise with defined hospitals in the health board and specific care homes to collect information on bed availability.
- Support ward staff to maximise timely patient discharge through effective discharge referral management.
- Will be required to assess and make judgements on a range of operational situations using critical evaluation approach to solve problems using a variety of solutions and Navigation Hub resources, seeking advice and guidance where necessary.
- Be ‘virtually’ accessible and approachable to ward based staff in the hospitals as well as local authority colleagues to ensure good relationships are formed.
- Engage actively with ward staff to reduce discharge delays relating to referrals and/or lack of available community care capacity and escalate concerns to the Navigation Hub Manager. This communication can be sensitive and cont
