The TBspecialist nurse will support the team leader for TB community nursing &Health Integration Team providing a comprehensive Barnsley TB serviceencompassing disease control, care & prevention

The teamadopts a multi-disciplinary approach, maintaining close links to all relevanthospitals, community agencies & Public Health England

You willbe required to assess adults & children for TB, screen with Mantoux orIGRA, promote HIV & other blood borne virus testing, undertake holisticassessments applying national & local guidelines, caseload manage, identifyclose contacts of index cases & facilitate appropriate screening

There isa requirement to visit adults, children & families with active TB at homeacross Barnsley during their treatment, including those with MDR/XDR TB, HIV& other co morbidities. There is a requirement to teach undertake serviceaudit & maintain IT literacy. Applicants should have good interpersonal& communication skills, & ability to communicate sensitive informationto patients/relatives where there may be barriers to communication orreassurance is required

Youshould be able to travel in a timely manner across Barnsley to undertake homevisits & deliver clinics

Main duties of the job

The TBteam provides specialist advice, leadership & clinical service in relationto TB

The goalsof the service are to identify & reduce the risk of people being newlyinfected with TB & provide high quality treatment & care for all peoplewith TB

The TBteam will achieve these goals by making all relevant groups of people aware ofthe facts of TB, strengthening more active case finding amongst high-riskgroups including contract tracing & ensuring everyone expected of havingthe disease has rapid access to TB services

Theseservices are needed by a range of groups with differing needs & deliveredin a variety of settings including community, clinics, patients homes & theacute sector. This is a flexible role to provide care to individuals, families& communities in response to changing local need

The postwill be based in Barnsley with close links to all relevant hospitals, communityagencies & the health protection agency. You will act as a positive rolemodel for all nurses through demonstration of expert clinical practice in thearea of TB.

Job responsibilities

JOB SUMMARY

The post holder will be a member of South West Yorkshire Partnership NHS Foundation Trust (SWYPFT) TB Team. The post holder will work with and deputise for the Team Leader in the management of Tuberculosisin collaboration with the Consultant Respiratory Physician and Paediatrician and co-ordinate the care of TB patients.

To manage a defined clinical caseload, providing specialist support, education and complex case management to people with active or latent Tuberculosis.

Liaise with primary and secondary care colleagues, providing education and support, thus developing and maintaining high quality evidence-based Tuberculosis care across Barnsley.

KEY RESULT AREAS:

Duties and responsibilities of the post

The postholder will:

Clinical Duties

Provide specialist advice and on-going support to patients with suspected or confirmed Tuberculosis and their families, including home visits. Provide specialist advice and guidance to other health professionals and external agencies concerning TB. Participate in health promotion activities on all aspects of TB. Supervise core and non-core staff and/or students. Actively contribute to the development and improvement of the service with the TB team and TB service users. Deliver TB screening of new entrants when required. Carry out and interpret the results of specific tests relating to TB. Recognise, investigate and take the necessary action to resolve problems in relation to noncompliance of TB treatment. Provide phlebotomy skills for Quantiferon testing in contact screening settings.

The post holder is a key member of the TB team whose overarching goal is deliver quality clinical and nursing care and TB prevention and control. This includes undertaking any activity deemed to be within the expectations of this role.

Professional Duties

To be aware of and act in accordance with Trust Clinical Standards and Guidelines and the NMC Code of Conduct and Guidelines maintaining competency and attending training as appropriate

Maintain and update own clinical specialist knowledge.

Ensure that all other SWYP NHS Foundation Trust policies and other regulations are applied as appropriate including Health and Safety for their own and others health and safety in the working environment and diversity.

Participate, through individual performance review, in matching organisational aims with personal objectives.

Communicate with patients and colleagues on TB transmission, prevention and management

Communicate information to those people who have the right, and need to know, consistent with legislation, policies and procedures.

Communicate at a level and a language (interpreting / advocacy) appropriate to the patient and their family/carers.

Keep accurate and complete records consistent with legislation, policies, and procedures.

Managerial duties

On occasion deputises Team Leader when they are not available

Partnership and Cross Boundary Working

Work with health, social and voluntary sector organisations, and staff with an interest in TB where necessary

Liaise with other professional services involved in the care of the patient, and where appropriate encourage the patients engagement with these services.

Attend and where appropriate, contribute to meetings, forums and case conferences including Cohort review and local TB network meetings.

Communication

Promote awareness of TB among local health and social care professionals and encourage them to rapidly refer all suspected cases for investigation.

Have collaborative relationship with Public Health England understanding their roles and responsibilities.

Communicate with people in a form and manner:

- Is consistent with their level and understanding, culture, background and preferred ways of communicating

- Is appropriate to the purpose of the communication and the context in which it is taking place

- Encourage the effective participation of all involved

Information Management

Has sound knowledge of all documentation and reporting requirements for effective case management.

Ensure notification of new cases and records outcomes of all assigned patients started on treatment.

Ensure all information on assigned patients required for cohort review is collected and entered correctly.

Undertake complex and detailed information analysis as required.

Substantial post qualification experience working in a clinical setting (acute or community). Possess effective communication both written and verbal is essential. Good standard of literacy, ability to write reports. Motivation and enthusiasm. Good time management and prioritising workload. Ability to work without supervision. Able to work on own initiative, organising and prioritising own and others workloads to changing and often tight deadlines. Demonstrate a high degree of reliability.

Desirable

Evidence of IT skills. Evidence of teaching skills. Evidence of further training or significant experience in TB or infection control or Public Health.
