Weare looking for an enthusiastic, passionate & committed Team Leader to jointhe innovative Barnsley Health Integration Team andBarnsley and Wakefield TB Services

Youwill be supported by the Neighbourhood Network Manager to ensure provision ofhigh quality, integrated & well-co-ordinated services

Youwill be an accomplished team leader with responsibility for day-to-daysupervision & line management of nursing staff & other team members. You will also be responsible for carrying out comprehensivehealth assessments to asylum seekers, refugees & other ethnic minoritygroups & marginalised groups,TB screening & treatments to peoplewith active & latent TB

Youwill be involved in the development of protocols, procedures, guidelines, carepathways in relation to clinical practice & working in partnership withhealth, social care, public health & third sector organisations &professionals

Main duties of the job

You will work in a variety of clinical & non-clinical settings, & represent SWYPFT at local, regional, & national forums & meetings

As Team Leader you will:

Provide specialist advice, leadership, & clinical service in relation to TB and Health Integration Have close links with all relevant hospitals, community agencies & the health protection agency Have good interpersonal & communication skills & ability to communicate sensitive information to patients/relatives where there may be barriers to communication or reassurance is required Supervision of staff to deliver an efficient & effective service Ensure high standards of care are delivered & maintained. This role requires resilience & ability to problem solve quickly & effectively Support quality clinical leadership Provide clinical supervision facilitating a culture of learning/reflective practice Have a key role in training, education, & staff development Have line management responsibility for staff with health & wellbeing a key consideration

Experience of working with people from different cultural backgrounds & with interpreters is advantageous

A current driving license and the use of a vehicle on a daily basis is essential

Job responsibilities

JOB SUMMARY

To carryout comprehensive health assessments, TB screening and first contact care in askilled and competent manner to Asylum Seeker and other ethnic minority groups. Towork closely with both the Neighbourhood Network Manager to ensure provision ofa high quality, integrated and well-co-ordinated service. Toanalyse and act on resource and caseload management issues, including forlatent and active TB cases, providing timely new entrant screening andnotification of active cases and contact training, using the relevant data andboth local and government targets. Tobe proactive in the support of government targets and maintain good workingrelationships with partner agencies. Communicate with patientsand colleagues on TB transmission, prevention, and management. Provide advice,information, and training to a varied cohort. Responsible for theday-to-day management / supervision of the team

CORE DUTIES ANDRESPONSIBILITIES:

Professional

Responsibilityfor service provision ensuring accountable practice whilst maintaining andenhancing current quality standards and delivery of performance targets. Toplan and manage own time/workload effectively balancing competing demands andfrequent interruptions whilst being available throughout the day for advice andsupport to junior staff and colleagues. Tocarry out effective quality health assessments in a manor that encourages thedivulgence of diverse and sensitive information in difficult situations. Toundertake comprehensive health assessments utilising highly developedspecialised knowledge and clinical expertise, which is theory-based andunderpinned with substantial practical experience in a competent manner. To identifyother screening or therapeutic interventions, signposting to appropriateservices as required, ensuring clients health needs are addressed. Provideinterventions and or treatments for TB which may include directly observedtherapy and contract tracing. Toprovide operational leadership to the team through periods of change, ensuringthe skills of the individuals meet the needs and expectations whilst acting asa positive role model to raise morale and innovative practice. Toattend operational meetings with partner agencies Department of Health andUK Border Agency and be proactive with any new health initiatives that may beintroduced. Identifythe individual potential in all grades of staff within the team and supportthem through undertaking effective annual staff development review processes. Beresponsible for the day-to-day management / supervision of the service ensuringadequate staffing levels are maintained and a high standard of care provisionare achieved to client expectations. Tobe involved in the development of protocols, procedures, guidelines, carepathways in relation to clinical practice, working in partnership with partneragencies as required. Identifyinghousing issues which impact on the delivery of effective TB services across thecaseload and raise these with local housing authorities, both directly andthrough health and well-being boards Tobe aware of and act in accordance with the Trust Clinical Standards andGuidelines and the NMC Code of Conduct. Toprovide cover and support to the Neighbourhood Network Manager as required andto deputise at meetings as appropriate Tosupply/prescribe medication according to the clinical assessment of theclients presenting condition; keep accurate and immediate records of everycontact.
