Job summary

To work within the Children's Community nursing team as an autonomous practitioner. Working alongside the MDT with families who's child has a life limiting condition, palliative diagnosis or are on an end-of-life pathway.

Facilitating conversations concerning hopes and wishes for children, young people and their families. Working in partnership with families to complete and regularly update ReSPECT documentation, often working alongside external agencies who provide palliative and end of life care to children and young people receiving care locally.

Main duties of the job

Working alongside colleagues to initiate the challenging conversations concerning palliative care and ReSPECT planning, with families, support them to have discussions and plan for their child's future.

To complete holistic assessment of the CYP nursing needs, developing care plans, including advanced care planning for end-of-life care provision

To support the provision of evidence-based care for children with a life limiting conditions in partnership with children, families and MDT including non NHS providers.

To communicate effectively with CYP, parent/carers and the wider MDT to ensure continuity of care, to empower families to discuss their wishes and plans

Demonstrate an awareness of safeguarding issues, be able to assess risk and refer to professional teams as appropriate.

Participate in team meetings, sharing knowledge and ideas

Qualifications/training & professional development

Essential

Registered Nurse (either RSCN or RN) with experience of working within Children's Services Evidence of ongoing professional development including Paediatric Palliative care

Desirable

Registered Nurse Child, RSCN registered with NMC Educated to degree level Teaching and Mentorship course

Experience

Essential

Significant experience working at a Band 6 in a community nursing environment Experience of working with children with a life limiting diagnosis and end of life care

Desirable

Previous experience of formal/informal teaching of patients, families, and staff Experience of working with children with complex healthcare needs

Skills, abilities & knowledge

Essential

Good theoretical knowledge and practical knowledge of nursing within the clinical area of paediatrics, committed to family centred care Ability to assess, plan, implement and evaluate care. Working alone and as part of a team Understanding the principles of clinical governance and clinical supervision Able to work flexibly within the multi-disciplinary team Current driving licence and ability to drive to a variety of rural/urban locations
