Job description

Job responsibilities

To provide an urgent 2-hour response to patients in the community to prevent hospital admission.

To undertake assessments and coordinate and provide therapy intervention to facilitate and support a safe discharge within a 24-hour timeframe.

To undertake specialist clinical therapy, including work requiring expertise or ability.

To promote integrated, multidisciplinary working practices that supports seamless care.

To support the Physiotherapy Team Lead, with service development, helping to implement policy changes and to set and monitor standards of practice.

To participate in health promotion and education of staff, clients and families

Person Specification

Qualifications

Essential

BSc / MSc in Physiotherapy. Current registration with HCPC Postgraduate qualification or evidence of relevant post-qualification professional development

Desirable

Specialist postgraduate training Practice Student Educator

Experience

Essential

Broad range of Band 5 rotations Previous experience of training/supervision. Experience of multi-professional working.

Desirable

Experience in acute setting. Experience in community setting

Skills

Essential

Evidence of CPD in previous year Knowledge of client centred practice

Desirable

Specialist client group knowledge.
