Contract - Permanent Full-Time 37.5 hours per week

Please note: All staff are required to be doubled vaccinated especially for clinical posts.
Main duties of the job
The recent CQC report rated the Whittington NHS services as 'Good' with 'Outstanding' for caring.
District and Community nurses visit people who are housebound of all ages 18+ in their own homes or residential homes, enabling them to self-manage where possible and implementing appropriate treatments and care pathways. Our service operates on a 24 hour basis so if you are looking for a working pattern that suits your life pattern. We currently have Deputy Team Manager posts in our day and evening teams and offer part time, full time and flexible working patterns.
This is an excellent opportunity for nurses who have met their competencies at band 5 level. Those who have already worked in District Nursing or would like to make the move to the community setting and further develop both their management and senior clinical skills. You will need to be self-motivated, an excellent communicator, both a leader and a team player with the drive and determination in case management and supporting the team manager to lead delivery of high quality nursing care for your team within an integrated care organisation.
All successful candidates will have the opportunity to develop skills in nurse prescribing, leadership and management as well as ongoing training, support, clinical supervision and a clear career pathway, with support from their team manager and Lead District Nurse and the District Nursing Professional Development Team.

Detailed job description and main responsibilities
MAIN DUTIES
Managing Care
To have responsibility for the effective and efficient delivery of nursing care to patients as delegated by District Nurse Team Manager.
To undertake skilled nursing activities directly related to the assessment, planning and evaluation of patient care and the supervision and training of junior staff and students.
To promote personal, professional, and clinical expertise through regular evaluation and updating of practice.
To ensure that practice is delivered in line with the NMC code at all times.
To ensure that the 6Cs (including compassionate care) underpins own practice and is promoted as an ethos within the service.
To assist in leading the team to meet the mission statement of the service and embed it into practice.
To fulfil requirements of integrated working and care, through effective liaison with Social Service Departments, Local Authorities and providers of acute care plus other agencies. To undertake health promotion, including chronic disease management, for patients and their families on the current caseload.
To identify any trained nurse within the team to be the named nurse for patients of the caseload.
To be involved in care/case management under the supervision of the District Nurse Team Manager, including of clients with continuing care needs.
Managing Resources
To teach and supervise the clinical practice of team members and to appraise and to identify training needs of all directly managed staff. In conjunction with the District Nurse Team Manager, meet the training needs of staff.
In conjunction with District Nurse Team Manager, identify the health needs of caseload to ensure the nursing team resources are deployed in a way that reflects those needs.
To promote good communication, employment practices and industrial relations within the nursing team.
To lead team (and other) meetings where appropriate.
To ensure the successful implementation of appropriate organisational arrangements within the team, so that the overall aims and objectives of the service are achieved.
To manage resources to ensure they relate to the overall objectives and responsibilities of the team.
To comply with financial standing instructions.
To be able to use the computer to: write written reports in Word, access/send emails on the Whittington Health system, access the intranet/internet for relevant information according to Whittington Health policy.
To be prepared to undertake training in using Protocol, the Whittington Health/SS joint single assessment process computer held document.
Record Keeping
To ensure accurate, contemporaneous records are kept that ensures safety and continuity of patient care (including electronic records where appropriate.
To ensure accurate statistical data is promptly submitted in line with Whittington Health Policies.
Quality
To participate in policy and standard setting, quality initiatives and clinical audit.
To be fully aware of the clinical governance agenda.
To ensure that the care given meets the standards and policies set by Whittington Health.
To inform line management of constraints that will prevent these standards from being met.
To respond promptly to any incident where service quality has fallen below an acceptable standard and report to the District Nurse Team Manager.
To attend regular clinical supervision and promote and support team attendance as per Whittington Health policy, as well as acting as a clinical supervisor.
To appraise junior members of the team using the Knowledge and Skills Framework
Whittington Health is an integrated care organisation and there may be occasions where staff are required to rotate between clinical environments to meet service need. This could be on a long or short term basis.
Professional development and responsibilities
Where the post holder has particular expertise, act as advisor/resource person to other staff.
To participate in training as appropriate of students nurses, NVQ candidates, GP trainees and other trainees.
To participate in resource projects and health promotion programmes as requested.
To undertake duties as delegated by the District Nurse.
Research
To participate in agreed research projects and/or innovations subject to discussion with the District Nurse and following agreed policy and protocol.
To contribute to the development of the nursing team and the community health services by participating in working parties, committees and planning and discussion groups
