Job summary
An exciting opportunity has arisen for a Registered Advanced Nurse Practitioner with an all round skillset to join our team at Hill Top Medical Centre.
We are a friendly, hardworking newly registered surgery, with 30 staff and around 9,800 patients, looking for an ANP to join our excellent nursing team.
Main duties of the job
You will be joining the existing team of nurses and HCA within the practice who undertake care of our patients. The ideal candidate will already have extensive ANP experience, preferably in a primary care setting , however consideration would be given to candidates with experience in secondary care, wishing to transition to primary care environment. You will make the best use of clinical skills within the practice in order to promote well-being and maximise health gain for patients and provide a safe, high quality and accessible service to our registered patients, many of whom have complex needs.

Job responsibilities
The Advanced Nurse Practitioner works autonomously and is accountable for own professional actions.
Undertakes Clinical Nursing Practice at an advanced level using expert knowledge and clinical skills to deliver holistic care to people accessing primary health care services.
Assesses diagnoses and treats patients attending primary health care services with a range of acute, non acute and chronic medical conditions.
Assesses diagnoses and treats patients in surgery or own home who require acute medical attention and refer to appropriate agency.
Directly admits patients to secondary care hospital in acute medical need.
Refers patients to other medical specialities for assessment if deemed appropriate.
Refers patients as appropriate to other members of the multidisciplinary team, secondary care and to other statutory and voluntary organisations including referral for x-ray.
Main Duties and Responsibilities :
1. Accountable for own actions in accordance with the NMC Code of Professional Conductand Scope of Professional Practice.
2. Act as a role model to nursing staff by delivering high quality care.
3. To manage and organise a specific caseload of patients and prioritise workload.
4. To assist patients to achieve individualised goals to maintain theirphysical wellbeing.
5. To advise medical and nursing staff on the most appropriate treatment for their patientbased on clinical assessment and clearly defined policies and protocols.

Qualifications
Essential
- Registered Nurse with additional Masters level Advanced Practice qualification. - Qualified independent prescriber able to undertake medication reviews if required. -Good organisational and communication skills. - Experience with audit and able to lead audit programmes. - Experience with clinical risk management. -The ability to both work under their own initiative and follow directions.
