About our FCP service

Our service has progressed and grown sinceits initiation in 2019. We are continually evaluating and redesigning ourservice to respond to the ever-changing demands within primary care. We have developed a bespoke FCP triageservice alongside the GP systems, to ensure we offer a true first contactservice for MSK conditions presenting to the surgery.

We work with extremely supportive GPcolleagues who recognise and appreciate our MSK expertise and value theautonomous FCP role our team offers. Wehave structured monthly governance sessions with our FCP GP Lead and have regularFCP team meetings, the team has a combination of portfolio and HEI FCPverification.

We are looking for an enthusiastic,well-motivated colleague who is either already working as an FCP or hasprevious MSK outpatient experience and is ready to join our very experienced andsupportive FCP team. We are a team of four staff (including this post) coveringtwo GP surgeries in the area. We are very proud of the unique FCP service weoffer our local population and are all excited to welcome a new colleague.

The successful candidate will befully supported within the team and mentored primarily by the FCP Lead. Allmembers of the team are keen to support a new colleague within the service tomanage risk and work as an autonomous practitioner. We are committed to supporting the FCPverification process.

Main duties of the job

Job Purpose:

Provide clinical expertise, acting as first-contactphysiotherapist and making decisions about the best course of action forpatients' care (including in relation to undifferentiated conditions). Thiswill involve triaging patients, without prior contact with their GP, in order to establish a rapidand accurate diagnosis and management plan.

Progress and request investigations to facilitate diagnosis andchoice of treatment regime, understanding the information and limitationsderived from these and the relative sensitivity and specificity of particulartests including diagnostic services such as x-rays and blood test, be able to interpretand act on results to aid diagnosis and facilitate the appropriate managementplans of patients.

Deliver programmes of supported patient self-management, in waysthat facilitate behavioural change, optimise individuals' physical activity,mobility, fulfilment of personal goals and independence,that could lead towards minimising the need for pharmacological interventions.

Participate in utilising the PCNs triage system KLINIK/Accrux and GPsystem EMIS to manage patient data and workload.
