**Job overview**:
The Consultant Practitioner will be in the unique position of providing strategic leadership across the
whole stroke pathway in Cornwall encompassing both the acute and community Trusts, across both
bedded and community care. They will lead on service transformation across both Trusts and build
on the existing collaborative work across the pathway to support the further development of a cross
organisational pathway approach to stroke care. They will provide a clinical lead across the pathway
driving the delivery of evidence based best practice.
The Consultant Practitioner will be a highly competent, visible and experienced autonomous
practitioner, capable of functioning effectively across key role domains (expert practice; leadership
and consultancy; education and training; service development, research and evaluation). They will
use their expert knowledge and skills to lead and deliver safe and clinically effective, quality driven,
personalised care for people across the whole system stroke pathway.

**Main duties of the job**:

- Autonomously act and be recognised as a clinical expert in their specialist field using highly specialist clinical knowledge ensuring patient / client centred care and family involvement, including health promotion and disease / accident prevention
- Manage a caseload that will often be complex and unpredictable: receive referrals, assess, order, interpret and act upon investigative tests, consider differential diagnosis, evaluate, treat, plan, refer and discharge patients within the Trust
- work with internal and external stakeholders to further develop and deliver a service specific pathway model of care for Cornwall and the Isles of Scilly
- provide highly specialist expert advice to develop services within the Trust and in association with agencies locally, regionally, nationally and internationally
- Collaborate with other researchers and secure funding to support specific research and practice development projects
- Publish and present developments and research locally, nationally and internationally
