We are looking for Mental Health Practitioners to work within our Primary Care Network, to lead our Mental Health and Well-being team, working alongside our Clinical Director, GP Mental Health Lead and Primary Care Network Manager. You won’t be on your own.

We are developing the team from the bottom up, health and wellbeing coaches, care coordinators and social prescribers are already in post. You can help us put in place the latest models of care for our community.

You will be instrumental within the team, managing the work, implementing and shaping new ways of working. Your role will involve liaising across practices and secondary care teams to ensure we are supporting our patient population in the best way we can. Based in practice or in our community hub you will help develop and implement effective systems to manage workloads.

**Main duties of the job**:
You can be full time part time or job share considered. We aim to create a space where we understand ‘what matters to someone’ not ‘what is the matter with someone’. Our new team will include a diverse range of people all there to listen, triage and help with aspects of a person’s life that may adversely affect their mental wellbeing.

The aims of this role will be to
- Deliver improved access to a broader range of care options by offering timely and expert assessment/triage
- Aim to include evidence informed approaches that are known to impact on both mental and physical health
- In keeping with the national guidance on the introduction of this role, it is envisaged that the focus of a practitioner’s work will generally be people whose needs cannot be met by IAPT (Outlook South West) service
