Main duties of the job

The discharge team at University Hospital Southampton NHS Foundation Trust (UHS) are delighted to be able to offer a fantastic opportunity for a focused, friendly and enthusiastic individual, to join as a Complex Discharge Operational Lead as part of our expanding service.

Provide professional and managerial leadership to the Complex Discharge Team. Be the operational lead and respond to the daily demand of the service. Support the Trust in its daily discharge processes.

Job description

Job responsibilities

To support the Complex Discharge Manager in the daily operational running of the UHS Integrated Discharge Bureau (IDB) team to ensure that the team deliver timely discharge of complex and delayed transfer of care (DTOC) patients including delivery of KPIs.

To support the Complex Discharge Manager in the delivery of the Hospital-to-home workstream, working with health and social care system colleagues to reduce the length of stay for and overall numbers of DTOC patients. This will include the delivery of assigned actions and projects.

To line manage the Continuing Health Care and Complex Discharge Team Leads in all areas of their roles.

To work closely with patients, families, wards and UHS departments to improve patient flow and prevent patients from becoming delayed in the discharge process, unblocking barriers when they occur.

To actively manage the discharge planning process for the most complex patients, specifically those with bespoke commissioning needs, multiple enhanced needs and those requiring an advanced clinical reasoning problem-solving approach.

To proactively escalate and problem-solve any barriers to the Complex Discharge Manager daily, offering solutions, suggestions and action plans where appropriate.

Masters level of education or can demonstrate an equivalent level of knowledge and skills through relevant experience in either a clinical or operational role

Extensive experience in the acute healthcare setting in clinical leadership Experience of introducing change to clinical environment Evidence of continuing professional development Ability to prioritise work and adapt to changing deadlines Able to develop projects from initiation to completion Articulate with highly developed communication skills Build relationships at all levels, including interpersonal skills, liaison and negotiation skills. Ability to get to place of work to include travel to all four UHS sites and external organisations
