About the Role

University Hospital Southampton NHS Trust is seeking a dedicated and experienced Complex Discharge Operational Lead to join our expanding Integrated Discharge Bureau (IDB) team. This role offers a unique opportunity to make a real difference in the lives of patients by ensuring timely and efficient discharges for complex cases.

Your Responsibilities

  • Provide operational leadership and management to the Complex Discharge Team, overseeing daily operations and responding to service demands.
  • Collaborate with healthcare professionals across various departments to facilitate smooth patient transitions and minimize delays in discharge processes.
  • Develop and implement strategies to optimize patient flow within the hospital system, focusing on reducing delayed transfers of care (DTOC) cases.
  • Line manage and mentor Continuing Health Care and Complex Discharge Team Leads, providing guidance and support for their professional development.
  • Proactively identify and address barriers to discharge, implementing solutions and action plans to ensure timely patient transitions.
  • Manage the discharge planning process for the most complex patients, requiring specialized knowledge and advanced clinical reasoning skills.

We are looking for an individual with a strong background in acute healthcare settings and proven experience in clinical leadership. You will possess excellent communication, interpersonal, and problem-solving skills, along with the ability to work effectively under pressure.
