Job Summary

We are seeking a highly skilled and dedicated individual to join our team as a Discharge Officer. As a key member of our Acute Discharge Hub, you will play a vital role in ensuring the safe and effective discharge of patients from our hospital.

Key Responsibilities
  1. Assist Patients in Transition: Collaborate with patients, families, and healthcare teams to facilitate a smooth transition from hospital to community care.
  2. Advocate for Patients: Act as an advocate for patients to ensure their discharge needs are met, and any barriers to discharge are addressed.
  3. Coordinate Discharge Planning: Work closely with multidisciplinary teams to develop and implement discharge plans, ensuring timely and effective transitions.
  4. Reduce Length of Stay: Proactively identify opportunities to reduce length of stay and delayed discharges, working closely with clinical teams and other stakeholders.
  5. Support Discharge Hub Team: Collaborate with the advanced discharge officer, complex discharge practitioner, and senior managers to ensure seamless discharge planning and escalation of complex cases.
  6. Liaise with Care Agencies: Establish relationships with care agencies and care homes to ensure timely assessments and transfers, working closely with social services, CCGs, and trusted assessors.
  7. Escalate Delays: Identify and escalate any delays within the discharge planning process to appropriate team leaders, clinical leaders, and service managers.
  8. Communicate with Bed Managers: Ensure bed managers are aware of medically fit patients and potential discharges, facilitating efficient bed management.
  9. Participate in Rotas: Participate in weekday, weekend, and bank holiday rotas as rostered, demonstrating flexibility and commitment to the team.
