Job Summary

We are seeking a highly skilled and dedicated individual to join our team as a Discharge Coordinator. As a key member of our Acute Discharge Hub, you will play a vital role in ensuring the smooth transition of patients from hospital to community care.

Key Responsibilities
  1. Advocate for Patients: Act as an advocate for patients to ensure their discharge is safe and effective, working closely with the multidisciplinary team to identify and address any barriers to discharge.
  2. Discharge Planning: Develop and implement effective discharge plans, working with patients, families, and healthcare professionals to ensure a seamless transition to community care.
  3. Communication and Liaison: Liaise with care agencies, care homes, and other stakeholders to establish dates for assessments and agree transfer dates, ensuring that patients receive the necessary support and care in a timely manner.
  4. Problem-Solving and Escalation: Identify and escalate any delays or issues within the discharge planning process, working with team leaders, clinical leaders, and service managers to resolve problems and ensure that patients receive the care they need.
  5. Collaboration and Teamwork: Work collaboratively with other members of the Acute Discharge Hub team, including advanced discharge officers, complex discharge practitioners, and senior managers, to ensure that patients receive a high-quality service.
