Job overview

We are recruiting for a Patient Pathway Coordinator who shares our vision to be trusted to provide consistently outstanding care and exemplary service to our patients.

Within this role you will be responsible for the monitoring and tracking of all patients within the specialty. The post holder will be responsible for ensuring all aspects of patient choice are met and will ensure that patients are treated in a timely way.

Main duties of the job

To work closely with the Service Coordinator, administrative colleagues, Clinical teams and other specialties within the division to ensure patients are actively supported through patient pathways.

To monitor patient’s pathways from referral, through treatment planning and treatment, escalating and averting any potential delays in the pathway

Liaise with members of individual specialties management teams and ensure coordination of all elements of patient pathways.

Ensure all information relating to the patient’s pathway is available to aid other users of the relevant systems, such as PAS, RTT, Chemocare, Qlikview etc.

Assist/cover areas of administration during busy periods, sickness and annual leave.

Working in a team environment to provide appropriate support/advice to others

Qualifications / TrainingEssential criteria
