Job summary

This is an exciting, rare opportunity for an experienced social worker to join our innovative and dedicated Patient and Family Support Service.

We are looking for a qualified social worker, who has experience of working with Adults, Children and Young People, to provide support in our In-Patient Unit and in the community. The postholder will be the Adult Safeguard lead for St Clare Hospice and will jointly lead the Adult and Children and Young People Safeguard initiative.

To ensure we support as many patients, relatives, and friends as possible, the post holder will be required to work 11am to 7pm / 12pm to 8pm at least once per week.

Main duties of the job

To provide specialist palliative social work tohospice patients, their families, and carers#

To work as part of the multidisciplinary teams inthe in-patient unit, therapy, and community team

To provide psychosocial social work as necessary to patients,family members and carers under hospice care

To assist in developing and growing the HospicesChildren and Young People service to support those under 18 years of age whoare living with someone with a life limiting or terminal condition or who haveexperienced a bereavement

Job description

Job responsibilities

To be an integrated member of the multidisciplinaryteams in the inpatient unit, therapy, and communityteam

To jointly manage the psychosocial team, including volunteersand students

To work in close co-operation with other members ofthe Patient and Family Support Team (PFST) toprovide integrated specialist psychosocial palliative social work andbereavement support both internally and externally

To develop patient and family support in line withhospice strategy

To participate in the daily triage and assessment ofall patients referred into the PFST ensuring the safety and appropriateresponse time of all referrals

To make and sustain links with external agencies andsources of social support relevant to the provision of specialist palliativecare at St Clare Hospice

To jointly lead the Children and Young peoplesafeguarding initiative and support in the design, delivery, and management oftraining for all hospice staff and volunteers as well as Mental Capacity Act(MCA) and Deprivation of Liberty Safeguarding (DoLS)

To contribute to the delivery of training toexternal agencies and organisations

To provide workplace support or supervision to otherstaff members where needed

To assist in the provision of teaching and supportto nursing and other staff on issues such as social work skills and knowledge

To work closely with community engagement teammembers and volunteers to provide social supportive interventions, establishcommunity links and develop the service

To manage own boundaries and raise any concernsaround this

To participate in meetings, working groups andcommittees as necessary within the service

To support volunteers by providing training andon-going supervision in collaboration with team colleagues

To be committed to own continuing professionaldevelopment in accordance with professional registration requirements

To participate in the development of the Strategic plan and the specialist service at StClare Hospice

Children and Young People Safeguard LeadRole & Adult Safeguard Deputy

Advise and support the Senior Leadership Team indeveloping and establishing the hospices approach to safeguarding

Play a lead role in maintaining and reviewing thehospices plan for safeguarding.

Create policies, procedures, and safeguardingresources to be used throughout the hospice

Advise on safeguard training needs and development,sourcing external training where required and provide training whereappropriate

Implementing and co-leading the safeguardingsupporters programme as a way of sharing information and responsibility forsafeguarding across the hospice teams

Contributing and attending the Local West EssexLocal Operational Group (LOG) meetings for Children and Young People

Maintaining and developing professional competenciesin relation to Safeguarding

Proactive in attending high quality supervision(currently with the ICB) and any development, training, and professionalsupport available through relevant networks

Maintaining an awareness and providing aprofessional joint lead on activities related to the MCA and DoLS and theirimpact upon Safeguarding Adults work

Develop Quality Standards for Safeguarding Childrenand Young People and undertaking regular audit of practice within the hospice

Provide safeguarding advice, support and supervisionto all staff teams and volunteers across the hospice

Daily monitoring of documented safeguard concerns toensure processed and responded to appropriately

Managing safeguarding concerns, allegations orincidents reported within the hospice
