About the Role

We are seeking a skilled Social Worker to join our Adult Social Care team in West Essex. As a key member of our team, you will work with adults and their families who have complex social needs and are more vulnerable due to disability, exclusion, or diminished capacity.

Your primary responsibility will be to maximize the potential of these individuals to live independently. You will work closely with our multi-disciplinary teams to provide a supportive framework for vulnerable adults.

Key Responsibilities
  • Chair meetings involving clients, family members, carers, and other multi-agency professionals to develop strong relationships with partner agencies and the local community.
  • Provide comprehensive assessments and develop care plans that meet the needs of vulnerable adults.
  • Work collaboratively with other teams to ensure a seamless service delivery.
  • Stay up-to-date with current legislation, regulations, and guidance to ensure best practice.
Requirements
  • Qualified Social Worker with a current Social Work England registration.
  • At least 1 year's social work experience, with evidence of continuing professional development.
  • Capability of practice in accordance with current professional standards.
  • Comprehensive understanding of current legislation, regulations, and guidance.
  • Ability to make clear decisions and highlight potential risks associated with intervention.
