About the Role

We are seeking a skilled Social Worker to join our Adult Social Care team in West Essex. As a key member of our team, you will work with adults and their families who have complex social needs and are more vulnerable due to disability, exclusion, or diminished capacity.

Your primary responsibility will be to maximize the potential of these individuals to live independently. You will work closely with our multi-disciplinary teams to provide a supportive framework for vulnerable adults.

Key Responsibilities
  • Assess and develop care plans for vulnerable adults
  • Work with adults and their families to identify and address their social care needs
  • Collaborate with other professionals to provide a comprehensive support network
  • Chair meetings with clients, family members, carers, and other multi-agency professionals
  • Develop strong relationships with partner agencies and the local community
Requirements
  • Qualified social worker with a current Social Work England registration
  • At least 1 year's social work experience
  • Evidence of continuing professional development and capability of practice in accordance with current professional standards
  • Comprehensive understanding of current legislation, regulations, and guidance
  • Ability to make clear decisions and highlight potential risks associated with intervention
