Job summary

To promote expert, individualised patient centred clinical care for clients with compromised skin integrity and pressure ulcers by using evidence based practice for the population of Croydon Health Services (CHS). The pressure ulcer nurse will assess and plan care for patients referred to the service and work in conjunction with the tissue viability team. The role will include the provision of care and supervision the delivery of this care by clinical staff. The post holder will offer education and support to patients, carers and other health care professionals.

The pressure ulcer nurse will work closely with other health care professionals to promote a high standard of care in the prevention and management of pressure ulcers ensuring the pressure ulcer policy & pathway are followed. This in line with local and national guidelines and best practice in the aim to reduce all grades of pressure ulceration.

We require a full time Band 6 District Nurse/Case holder to work within our new Integrated Care Teams to provide efficient and effective nursing care to patients in their own homes. Excellent clinical, leadership and communication skills are essential along with the ability to work in an innovative way as part of the new Integrated Community ideal candidate must be a Registered Nurse with a Specialist Practice Qualification in District Nursing or have experience working as part of the Community Nursing candidates must have a clean full UK driving licence and the use of a car for work purposes.

Essential

Registered General Nurse-Adult. Specialist Qualification in area of Practice or significant experience in nursing adults in the community. Mentorship qualification. Willingness to undertake further training relevant to this post. Nurse Prescribing or willingness to undertake Live registration as a sign off mentor.

Relevant Knowledge/ Awareness
