Patient Pathway Coordinator Band 4

An exciting opportunity has arisen for an Patient Pathway Coordinator to join the Macmillan Team.

We are looking for a reliable and enthusiastic administrator to work within our Macmillan Team to ensure a proactive and efficient service. You will support the CNS teams as well as liaising with MDT co-ordinators to ensure the smooth and efficient day-to-day running of the GI Tract two week wait referral service. As part of this role, you will communicate on a daily basis with cancer patients, relatives, clinicians, MDT, and wider CUH staff, GPs and outside organisations.

**You need to be**:

- Able to work in a fast paced environment, where no day is the same.
- Able to provide a comprehensive administrative service to the Triage Assessment Team; with the ability to identify when to escalate clinical concerns to the nursing team, and operational concerns to the ANP and Lead Cancer Nurse.
- An excellent communicator; confident in receiving telephone calls and responding quickly to patient/ clinician/ GP enquiries, whilst being attentive to patient their needs.
- A committed team player, working collaboratively with all professional members of the CMT and wider CUH teams.
- Able to work independently, prioritising own workload and flexible in responding to patient/ clinician/ CMT enquiries, demands of the services and demands of the role.
- A quick learner, able to adapt quickly and use own initiative; and show assertiveness.
- Attentive to detail and able to retain and disseminate information in a clear and concise manner.
- A proficient typist, able to type letters for the clinicians within the colorectal nursing team.
- Experienced in using databases including the inputting, interpreting and presentation of data.
- Experienced in co-ordinating diaries and meetings, and compilation and distribution of agendas and minutes.
- Have an understanding of cancer pathways and waiting times.
- Experienced in working in a front line NHS environment.
