Job overview

We are looking for a Band Community Nurse Associate to work within the Peterborough Community Nursing Team supporting patients with clinical health needs.

This role will enable you to develop and enhance your clinical community skills to include wound management, discharge support, diabetes management, catheter management and those living with long-term conditions at home.

The post holder will provide quality evidence based nursing advice, support and care for service users with physical health needs and to their carers within a multidisciplinary setting. They will be responsible for the assessment, planning, implementation and evaluation of nursing care in line with the agreed NMC standards specific to the role, and will have a working knowledge of local, national policies and legislation which govern current service provision.

The post holder will be responsible for identifying and contributing to the training needs of junior unregistered staff, and ensure that issues of equality, diversity and rights are dealt with in accordance with good practice and legislation. They will ensure that effective communication pathways exist, and that they act within their sphere of responsibility.

Main duties of the job

Working as a member of the Community team undertaking delegated duties in order to meet the needs of the needs of the service users. You will be trained and supervised by registered professionals and will be expected to follow a plan of care based on the specific needs of the service users.

Experience of community nursing, palliative care, care of patients with diabetes Educated to Registered Nursing Associate on the NMC Register Experience of caring for and supporting adults and older people Experience of assessing patients and formulating care plans independently and where appropriate, with support from RNs in the team. The ability to demonstrate a commitment to high quality service provision The ability to work flexibly and as part of a team, as well as individually The ability to travel independently using own vehicle Good time management and the ability to manage individual workload Enhanced communication and inter personal skills Working for our organisation
