Job responsibilities

RESPONSIBILITIES

Clinical

To be professionally and legally responsible for all aspects of your own work as an autonomous practitioner, including the management of patients in your care.

To be responsible for the provision of specialist Occupational Therapy care for patients under the care of the Community Based Neuro Rehab Team. To ensure patients under your management receive a high standard of clinical care and support, and that other Occupational Therapy staff do likewise.

To work closely with the Occupational Therapy in-patient Senior Therapists & other neurological specialists and the wider team to ensure Occupational Therapy services provided are of a high standard, responsive to patient and multidisciplinary team (MDT) needs and to ensure the service is run in an efficient and effective manner.

To be responsible for transferring individual patients ongoing care to senior Occupational Therapy staff or other professions / agencies, as appropriate, by providing comprehensive assessment, treatment plans and undertaking joint treatment sessions as indicated.

To gain valid, informed consent from patients using the standards and guidelines provided by the College of Occupational Therapy (British Association of Occupational Therapists). To work within the legal framework, seeking appropriate professional assistance when the capacity to consent is in doubt.

To undertake comprehensive assessment of patients, including those with highly complex presentation, using investigative and analytical skills. To formulate individualised management and treatment plans, using clinical reasoning, knowledge of evidence-based practice and utilising a wide range of treatment skills and options to form a specialist programme of care in agreement with the patient and / or carer.

To use highly developed specialist manual therapy skills, including dexterity, co-ordination and sensory skills, in the assessment and treatment of patients and to continually reassess and re-evaluate patient presentation altering treatment programmes accordingly.

To use highly developed specialist skills such as Cognitive and Perceptual assessments and treatment of patients.

To provide highly comprehensive immediate and planned advice, teaching and instruction to relatives, carers, other Occupational Therapy staff, other MDT members, homecare and agency support staff and other professionals to promote understanding of the aims of specialist Occupational Therapy for patients with a neurological condition and to ensure a consistent approach to patient care.

To communicate effectively, highly complex and sensitive information, with patients, carers, guardians and medical practitioners/other professionals to maximise rehabilitation potential and to ensure understanding of their condition. Communication skills of persuasion, motivation, and explanation will be necessary with all patients. Barriers to effective communication will regularly be evident, altered perception, expressive and receptive dysphasia, poor motivation, language barriers, pain and fear.

To maintain, and ensure all staff maintain accurate, comprehensive and up to date documentation, in line with professional standards, trust guidelines and legal requirements. To communicate assessment, treatment and results of treatment to the appropriate disciplines in the form of reports and letters including timely discharge summaries for other agencies including GPs, Social Services and other professional colleagues.

To ensure clinical governance and risk management are integral to the service, including consideration of tasks given to support staff, and find effective and efficient ways to solve problems and report findings to relevant departments.

To be an active participant at MDT meetings, working closely with the team on joint goals, reviewing patient treatments, progress and plan future discharge. To use clinical experience and knowledge to act as a resource for the team when analysing and judging the appropriate treatment pathway for highly complex patients.

To communicate effectively and work collaboratively with Medical, non-medical, Nursing and Therapy colleagues and other agencies across Health / Social services to ensure delivery of a co-ordinated multidisciplinary service.

To be responsible for equipment used in carrying out Occupational Therapy duties. To assess, order and fit equipment and to adhere to departmental policy, including competence to use equipment and to ensure the safe use of equipment by others through teaching, training and supervision of practice.

To maintain a high level of professional responsibility, by following professional/clinical guidelines / standards, and Trust policies / procedures.
