Job summary

Main duties of the job

The post holder is an experienced nurse who, acting within their professional boundaries, will provide care for the presenting patient from initial history taking, clinical assessment, diagnosis, treatment and evaluation of care. They will demonstrate safe, clinical decision-making and expert care, including assessment and diagnostic skills, for patients within the general practice. The post holder will demonstrate critical thinking in the clinical decision-making process. They will work collaboratively with the general practice teams to meet the needs of patients, supporting the delivery of policy and procedures, and providing nurse leadership as required. In order to work at this level NMC requirements for advanced practice must be met.

Crawley Care Collaborative PrimaryCare Network is an NHS Collaboration between four GP Practices - Bridge MedicalCentre, Langley Corner Surgery, Leacroft Medical Practice and Southgate MedicalGroup - working together to provide enhanced access services.

Our surgery teams work closely, sharing expertise andresources to develop new services. Our vision is to continue to improve thequality of care that we provide in alignment with the needs of our patientpopulation.

Our Primary Care Network builds on the existing primary careservices and enables a greater provision of proactive, personalised and moreintegrated health and social care. We are supported by practitioners inadditional roles who allow us to create bespoke multi-disciplinary teams basedon the needs of our local population. Byworking together with local community services, this allows us to make supportavailable to people where it is most needed.

ClinicalPractice

Assess,diagnose, plan, implement and evaluate treatment/interventions and care forpatients presenting with an undifferentiated diagnosis. Clinically examine and assesspatient needs from a physiological and psychological perspective.

Assess,diagnose, plan, implement and evaluate interventions/treatments for patientswith complex needs. Proactively identify, diagnose and manage treatment plansfor patients at risk of developing a long-term condition.

Diagnoseand manage both acute and chronic conditions, integrating both drug- andnon-drug-based treatment methods into a management plan.

Prescribeand review medication for therapeutic effectiveness, appropriate to patientneeds and in accordance with evidence-based practice and national, local andpractice protocols, and within scope of practice.

Workwith patients in order to support compliance with and adherence to prescribedtreatments. Provide information and advice on prescribed or over-the-countermedication on medication regimens, side-effects and interactions.

Prioritisehealth problems and intervene appropriately to assist the patient in complex,urgent or emergency situations, including initiation of effective emergencycare.

Supportpatients to adopt health promotion strategies that promote healthy lifestyles,and apply principles of self-care.

Communication

Utiliseand demonstrate sensitive communication styles, to ensure patients are fullyinformed and consent to treatment, including communication with, and supportof, patients and their significant persons who are receiving bad news.

Communicateeffectively with patients and carers, recognising the need for alternativemethods of communication to overcome different levels of understanding,cultural background and preferred ways of communicating. Anticipate barriers tocommunication and take action to improve communication.

Maintaineffective communication within the practice environment and with externalstakeholders

Act asan advocate for patients and colleagues.

Ensureawareness of sources of support and guidance and provide information in anacceptable format to all patients, recognising any difficulties and referringwhere appropriate.

Deliveringa Quality Service

Recogniseand work within own competence and professional code of conduct as regulated bythe NMC (Nursing & Midwifery Council, 2015c).

Produceaccurate, contemporaneous and complete records of patient consultation,consistent with legislation, policies and procedures

Criticallyevaluate and review innovations and developments that are relevant to the areaof work

Enlistsupport and influence stakeholders and decision-makers in order to bring aboutnew developments in the provision of services

Contributeactively to the PCN Board, attending all meetings that are relevant while beingthat voice for nurses in the PCN
