Advanced Nurse Practitioner

We are seeking an experienced Registered Nurse to join our team as an Advanced Nurse Practitioner in Children and Young People's Community Health. The ideal candidate will have a strong background in community nursing and be passionate about delivering high-quality care to children and young people.
Key Responsibilities:
- Develop and implement individualized care plans for children and young people, working closely with families and other healthcare professionals.
- Provide comprehensive assessments, diagnoses, and treatment of children and young people's health issues, including physical, emotional, and mental well-being.
- Collaborate with multidisciplinary teams to coordinate care and ensure seamless transitions between services.
- Maintain accurate and up-to-date patient records, ensuring compliance with trust policies and procedures.
- Stay current with best practices and evidence-based guidelines, participating in ongoing education and professional development opportunities.

Requirements:
- RN Child or RSCN qualification (or equivalent experience)
- Minimum 2 years' experience in community nursing, preferably with children and young people
- Strong clinical assessment and care planning skills, with ability to work autonomously and collaboratively
- Excellent communication and interpersonal skills, with ability to engage with diverse groups
- Post-registration professional development within the last 3 years
