Job summary

An opportunity for an experienced Rehabilitation Physiotherapist to work on Hugh Squier Ward at South Molton Community Hospital. Patients on the Ward are predominantly patients that are medically optimised and require rehabilitation to improve function. The key role is to optimise independence so that patients can return home.

Main duties of the job:

To provide Clinical and Operational leadership. To set patient centred goals, informed by evidence and best practice. To deliver Rehabilitation programmes to improve patient outcomes. To have a clear understanding of Trust and National drivers for the delivery of the service. To be involved in an active training programme. To be active in Service development, quality improvement, and clinical governance. All Physiotherapy staff are required to participate in the Respiratory on call or Rehabilitation weekend working rotas and take fair share of bank holiday working.

We are looking for a dynamic Physiotherapy Advanced Practitioner to work jointly with the existing Physiotherapy Advanced Practitioner on Hugh Squier Ward.

The post holder will be required to:

Ensure delivery of high quality patient centred care measured by clinical effectiveness, patient safety and patient experience in line with national guidance. To work as an autonomous practitioner, be up to date with evidence-based practice and be able to offer advanced assessment and treatment skills of patients. To be responsible for ensuring excellence with in the team by initiating and participating in evidence-based projects and audit, to improve both immediate and future quality of care.

As part of the role there will be a need to provide Advanced Physiotherapy advice to the team and other disciplines/agencies. You will need to be able to cope with demands on your time with frequent interruptions.

It is a requirement of the role to evidence post graduate training at a Master level or equivalent. You do not need to have completed a full Masters.

The Trust is keen to support staff to develop strong leadership and clinical skills through clinical supervision, internal and external continuing professional developments.

We are continuing to develop links with Universities to support staff with training and development.

Working Pattern: 30 hours per week. 08:00 to 16:00 over 4 days a week including on call and or weekend working.
