Job summary

The PCN Manager will support the PCN to develop and implement serviceimprovement and transformation work, aligned to local and national priorities.This includes building on current primary care provision and workingproactively with the wider network to ensure we can offer our local populationcoordinated and more integrated healthcare. The role entails building effectiveworking relationships, working closely with the Clinical Director, engagingstakeholders across different organisations and disciplines, coordinatingwork-streams, and collating and monitoring risks and issues.

The PCN Manager will coordinate the successful delivery of allcontractual requirements, translate national and local policy into meaningfulchanges, and will support the network to achieve improved outcomes for ourlocal population.

The PCN Manager will also be responsible for leading on digitaltransformation within the new PCN. This will involve close liaison on digitalmatters with the 4 surgeries and have a digital vision going forward.

The PCN Manager will bring strategic vision and oversight along withoperational skills. The role will be responsible for strategic delivery andhaving oversight of projects and services committed to, or commissioned by, thePCN. The role will oversee financial management, resourcing, and regulatorycompliance of the PCN work. As a memberof the PCN management team, you will support colleagues in PCN practices withchange management.

The PCN Manager will coordinate the successful delivery of allcontractual requirements, translate national and local policy into meaningful changes,and will support the network to achieve improved outcomes for our localpopulation.

You will have previous knowledge and experience of NHS working however,those with transferable skills may be considered.

About us

Forest Green PCN is a networkof 4 GP practices, with a patient population of over 29,000, in semi-ruralGloucestershire. The PCN is committed to maximising outcomes for the PCN DESfor our 4 surgeries and their patients. The surgeries include, Dockham Surgery,Drybrook, Forest Health Care and Mitcheldean. The Forest of Dean has its own unique challenges.Forest Green PCN is a new PCN formed after the split of the Forest PCN. The oldPCN was a trend setter in Gloucestershire with notable firsts including combiningEA with IA and we hope to continue that in Forest Green. We will be aninnovative and proactive PCN with a strong compliment of ARR staff including a PCNmedicines team consisting of pharmacy technicians and pharmacists, care coordinatorteam, childrens and family social prescribers and mental health worker. We arekeen to widen this further to support patient care and practices within theavailable resources and workforce capacity.

Job responsibilities

JOB RESPONSIBILITIES

Financial and contractual performance and financial reporting

Responsibility for PCN financial performance

Produce an annual statement of Income and expenditure for PCN auditors

Provide financial reports to network practices

Business Planning and PCN Strategy Development

Lead the development of a 5-year strategic plan.

Support change and monitoring practice PCN DES performance

Identify new income streams and new services

Support, develop and evaluate strategic development of all services

Apply for PCN development funds and resources and to support the network achieve its objectives.

Support PCN development nationally and advising the Clinical Director and member practices on initiatives, policy development, best practice, and funding streams.

Provide digital transformation leadership to the PCN.

Meetings

Support all PCN meetings, lead and chair as required

Act as PCN representative on countywide PCN Business Manager meetings and other relevant meetings with the IBC and other partners

Act as point of contact for the Clinical Director, practices, screening communication, correspondence

Encouraging and fostering effective working relationships between networks and within the ILP

Work with member practices to consolidate the network, promoting its ethos and values.

Attend county digital transformation meetings.

Bids/Contracts: develop, sign off and responses

Reviewing PCN service specifications, identify KPIs and key deliverables

Prepare and present Business Cases to the PCN practices

Working with member practices to ensure that all obligations under the agreement are met and network practices are fairly and equitably remunerated.

Overseeing performance dashboards and supporting practices to implement remedial actions where needed.

Representing the interests of the network at contract meetings with other bodies such as the ICB

Presenting data to member practices, PCN clinical leads, Clinical Director and other stakeholders related to network performance

HR/Workforce planning

Lead and manage HR, performance, development and appraisal of network staff with clinical support where required

Manage workforce returns as required by NHS E/ICB

Lead on all PCN workforce planning and proposals for PCN

Act as a line manager for relevant or senior ARR roles

Attend ARR staff team meetings as required.

Communication and Engagement

Maintain effective and timely communication with network practices, clinical director, and network employees.

Communicate with network practices, ICB and other stakeholders such as the GP federation, Care Homes etc and promotes the identify and ethos of the network.

Communicate effectively with public groups, patients, and carers.

Be comfortable and competent at working at all levels.

Enable and encourage sharing of good practice across the network helping to reduce duplication and wastage

Apply professional standards

Develop a PCN wide social media communication

Oversee alignment of digital options for service delivery across the PCN

Digital Transformation

Identify opportunities for transformation activities, utilising access, and PHM data to tackle inequalities and supporting practices to deliver change, improving systems for enhancing patient care.

Link with the ICB and ICS teams within Gloucestershire, as well as NHSE regionally and nationally to ensure PCN maximises opportunities available for QI, digital transformation to maximise funding and resources.

Plan, co-ordinate and directly support the delivery of transformation activity in general practice within the PCN with a view to improving patient access, quality and continuity of care, staff and patient satisfaction, and the efficiency and sustainability of general practice services.
