Job summary
An exciting opportunity has arisen to for a Band 6 Learning Disability Caseworker to join the Intensive Support Team on a fixed term/secondment contract to cover maternity leave.
We are looking for a keen team player who has a good understanding in the transforming care agenda.
The candidate should have knowledge around person centred care and positive behavioural plans, which incorporate low arousal approaches and promotes least restrictive practice; an understanding of Autism and how it can impact upon delivery of care as well as experience within a community setting.
The successful candidate will have experience working with Learning Disabilities amongst people with a complexity of needs and challenges.
The applicant should be able to work flexibly and with minimal supervision with the ability to cope with changing situations to routine.
The post holder will require a flexible approach to assist when and where required between the hours of 9am to 5pm Monday to Friday and occasionally to 8pm.
A range of additional training will be offered to supplement the role.
Main duties of the job
Work within a specialist clinical team providing a service to adults who have a learning disability, mental health and/or Autism and who present with behavioural challenges.
Provide support via assessment, behavioural interventions, risk assessments, relapse plans, positive behavioural plans and monitoring of service users needs.

Job description
Job responsibilities
Work with the Intensive Support Team, LearningDisability Leads and commissioners to support the use and management of theDynamic Support Register, identify those who require or who would benefit froma Care Treatment Review and develop a plan.
Contribute to the Dynamic Support At RiskRegister for adults who are at risk of being admitted to hospital. Provideclinical input to Red, Amber, Green (RAG) statuses as part of the register andliaise with wider professionals to ensure all relevant individuals are capturedon the register.
Promote national learning disability agendasincluding Transforming Care, Positive behaviour Support and mental healthrecovery models, STOMP, and Greenlight working.
Be aware of the need to protect individualsfrom abuse and harm in line with local safeguarding policies and procedures.

Good knowledge of working with people with a learning disability and their families and carers Good knowledge of making reasonable adjustments for people with learning disabilities and vulnerabilities A good level of knowledge of current policies and legislations that impact upon people with a learning disability A good level of knowledge in a wide range of common health care conditions in people with learning and other disabilities and vulnerabilities including common long term conditions Child protection and vulnerable adults procedures Knowledge of cognitive behavioural therapy
