About the Role
We are seeking a highly skilled and dedicated Community Health Specialist to join our Intensive Support Team.
As a Learning Disability Clinical Lead, you will work closely with the team to provide a service to adults who have a learning disability, mental health, and/or Autism and who present with behavioral challenges.
Key Responsibilities
- Providing support via assessment, behavioral interventions, risk assessments, relapse plans, positive behavioral plans, and monitoring of service users needs.
- Working with the Intensive Support Team, Learning Disability Leads, and commissioners to support the use and management of the Dynamic Support Register, identifying those who require or who would benefit from a Care Treatment Review and developing a plan.
- Contributing to the Dynamic Support At Risk Register for adults who are at risk of being admitted to hospital, providing clinical input to Red, Amber, Green statuses as part of the register, and liaising with wider professionals to ensure all relevant individuals are captured on the register.
- Promoting national learning disability agendas, including Transforming Care, Positive Behavior Support, and mental health recovery models, STOMP, and Greenlight working.
- Being aware of the need to protect individuals from abuse and harm in line with local safeguarding policies and procedures.
Requirements
To be successful in this role, you will need to have:
- Registered Nurses in Learning Disability (RNLD), Registered Mental Health Nurses (RMN), Diploma in Occupational Therapy (Dip OT), or Diploma in Social Work (Dip SW)
- Registered with the Nursing and Midwifery Council (NMC), Health and Care Professions Council (HCPC), or Social Work England (SWE)
- BSc in Community Health Studies or equivalent qualification
- Evidence of post-registration training/qualifications
- Demonstrable evidence of participation in continuous professional development
- Ability to meet the travel requirements of the role
- Good knowledge of working with people with a learning disability and their families and carers
- Good knowledge of making reasonable adjustments for people with learning disabilities and vulnerabilities
- A good level of knowledge of current policies and legislations that impact upon people with a learning disability
- A good level of knowledge in a wide range of common health care conditions in people with learning and other disabilities and vulnerabilities, including common long-term conditions
- Child protection and vulnerable adults procedures
- Knowledge of cognitive behavioral therapy
- At least two years post-registration experience working with people with complex health needs in the field of Learning Disability and/or Mental Health
- Working with external stakeholders/agencies, carers, and families
- Previous experience of supporting the needs of people with complex behavioral challenges
- Experience of conducting cognitive behavioral assessments
- Ability to praise and be supportive to others
- Ability to communicate clearly and effectively in English in both verbal and written formats
