Operated by a multidisciplinary team,including GPs, nurses, and other healthcare professionals delivering variousappointments depending on clinical need, including face to face and remoteappointments via telephone or online.
This is a consistent national access offer toprovide routine appointments at evenings and weekends to meet locallydetermined demand, alongside effective access to out of hours and urgent careservices.
Additionally, this provides a solution forpatients who may otherwise attend A&E.
Overall, this will enhance primary care accessacross Lincolnshire while also increasing the level of engagement andparticipation of all practices working together and closer to otherprofessionals from across the health and social care system
Main duties of the job
The Healthcare Assistant will assistmedical personnel in the care of organisation patients to include treatment,preventative care, screening and patient education. They will act as a focalpoint of communication between patients, doctors, nurses and other members ofthe primary health care team The post will also include some administrativeduties.
A uniform and name badge must be wornat all times.

We are a GP Federation covering 16 practices. 10 in Grantham and surrounding villages and 6 in Sleaford. We cover 130,000+ patients across all sites.
We are continuing to support our practices resilience through the Additional Roles Reimbursement Scheme (ARRS); and supporting and developing neighbourhood working across our population.
We have teams of Clinical Pharmacists, Pharmacy Technicians, Social Prescribers and Link Workers, Occupational Therapists, Care Coordinators and Paramedics.

Nursing:
BP monitoring
Urinalysis
Phlebotomy
Chronic disease reviews (physical health check)
NHS Health checks
Provide lifestyle advice to patients making any necessary referrals within the organisation.
Basic wound management (dressings, suture removals)
Weight monitoring
Ensure specimens are labelled and bagged ready for collection with the necessary paperwork completed.
Management of appointment system
Ensure total familiarity with all appointment systems including regular and incidental variations.
Book appointments and recalls ensuring sufficient information is recorded to enable retrieval of the medical record.
Monitor flow of patients into the waiting room ensuring the appointment system accurately reflects the arrival of patients.
Monitor effectiveness of the system and report any problems or variations to the senior receptionist.
