Job summary
:Working in our Primary Care Hub on behalf of a group of GP Practices, withina Multi-Disciplinary Team, you will support our team of Patient Advisors whoprovide and signpost same day care for the local patient population.
The main responsibilitiesof the post-holder would be:
Conduct patient triage and assessments to determine appropriatetreatment plans
Provide primary medical care to patients of all ages, includingdiagnosing and treating acute and chronic illnesses
Order and interpret diagnostic tests, such as blood work andimaging studies
Undertake a variety of duties, including surgery consultations,telephone consultations and queries, visiting patients at home
Collecting data for audit purposes;
Compiling computer-generated acute and repeat prescriptions forauthorisation by a Doctor
Collaborate with specialists and other healthcare professionals tocoordinate patient care
Educate patients on preventive healthcare measures and promotehealthy lifestyle choices
Maintain accurate and up-to-date medical records for all patients
Stay current with advancements in medical knowledge and techniquesthrough continuing education
Carryout and interpret diagnostic tests under supervision of a GP.
Independentlydecide on and carry out treatment working under the remit of local PGDs Assessand evaluate, with patients, the effectiveness of the treatment and careprovided.
Referpatients for investigations and/or the appropriate specialist where appropriateEnsure the provision of continuity of care including follow-up visits.
Takea holistic approach to patient care, bringing together their medical, socialand psychological needs.
Empower patients to take responsibility for managing their owncare and treatment, where it is safe and appropriate to do so.
Record the Consultation in the patient records in the EMISClinical System.
Contribute to and write Care Plans - For those patients at risk of hospital admission, for example , ina care home or with complex needs they have a care plan which is recorded inthe patients clinical record.
Essential skills:
Strong clinical skills with the ability to diagnose and treat awide range of medical conditions
Excellent communication skills, both verbal and written, toeffectively interact with patients, their families, and other healthcareprofessionals
Ability to work well under pressure in a fast-paced environment,particularly in critical care situations
Attention to detail and strong organisational skills to managepatient records and documentation accurately
Empathy and compassion for patients, demonstrating apatient-centred approach to care
Ability to work independently as well as collaboratively within ahealthcare team
Experienced in Primary Care, would considerparamedic/nurse practitioner
Registered with the appropriate Health andCare Professionals /Nursing Council
Backgroundof working in a fast paced Primary Health Care or acute environments
Provenbackground of dealing with a diverse range of patients from all walks of life.
Provenbackground of building positive rapport with a diverse range of internalcolleagues and clients
Provenability to work autonomously whilst also having an awareness when identifyingthe need to access the wider clinical team or external agencies appropriatelyand safely
Firstclass organisational and time management skills in a fast paced environment areessential.
Holda Full UK Driving License and ideally have access to a vehicle insured forbusiness insurance to travel between surgeries and other health care settingsas appropriate
