**KEY RESPONSIBILITIES**

The post holder will**:
1. Have an appropriate experiential health care background such as: health care assistant, therapy assistant, emergency medical technician.

2. Work with Remote Monitoring Clinicians supporting high level remote monitoring for patients with short and long term conditions. Escalate to Remote Monitoring Clinicians when markers for deterioration are evident on submission of abnormal observations by patients via the Docobo system. Thereby, ensuring patients on the caseload who ‘alert’ have clinical contact urgently. This ensures care is delivered in the community setting and aids prevention of unnecessary hospital admission.

3. Work on shared caseloads with Integrated Care System (ICS) by acting as an integral member of the community team with a shared vision for developing caseloads as required by Integrated Care System partners.

4. Possess experience working within the telemedicine field to support the delivery of clinical care remotely; working, under direction of the Remote Monitoring Team Leader and Remote Monitoring Clinical colleagues when required.

5. Deliver health promotion advice under supervision from clinical colleagues via the Remote Monitoring model, e.g. chronic disease management self-help signposting.

6. Work in alignment with Primary Care, for established practice populations, toward a shared vision of seamless management for patients with short and long term conditions within the Primary Care and the Community setting.

7. Work with ICS partners in Primary and Social Care to further develop the Remote Monitoring model for cohorts of patients managed within local populations.

8. Maintain accountability for practice to the Remote Monitoring Team Leader following all EBPC Policies and Procedures.

9. Maintain legible, accurate and up to date records that are dated and signed in accordance with EBPC Policies for standards on record keeping.

10. Contribute to the ‘onboarding’ of patients with short and long term conditions to the Docobo patient management platform.

11. Contribute to the ‘discharging’ of patients with short and long term conditions from the Docobo patient management platform.

12. Work in a supervised capacity with patients, their families and carers managing referrals according to need and priority.

13. Escalate complex clinical data that has alerted on the Docobo patient management platform to Remote Monitoring Clinical colleagues.

14. Understand and support the achievement of EBPC’s business plan objectives and performance targets.

15. Identify the potential for service developments, risks and deficits and inform the Remote Monitoring Team Leader making recommendations based on vocational knowledge.

16. Be accountable for practice taking every reasonable opportunity to sustain and improve knowledge and competence.

17. Assist the Remote Monitoring Team Leader to manage the effective use of resources within the Remote Monitoring team and participate in regular review meetings with the Team Leader.

18. Have experience of Docobo patient management platform or similar.

19. Have experience with other clinical patient management systems: Adastra, EMIS, Connected Care.

**PATIENTS CARE**

The post holder will:
1. Use telemedicine knowledge and skills to remotely monitor the physical and psychosocial needs of a defined client group.

2. Use telemedicine and healthcare knowledge to respond to alerts raised by the Docobo patient management platform. Urgently, escalating patient alerts to Remote Monitoring Clinicians to action.

3. Work with partners in nursing and residential care to improve the health outcomes of the residents and so prevent unnecessary hospital admissions or extended in-patient care episodes.

4. Assist Remote Monitoring Clinicians who are dealing with complex patient situations. Thereby aiding the establishment of a therapeutic relationship with individual patients using Remote Monitoring service.

5. Use telemedicine assessment tools/skills that will ensure an appropriate level of therapeutic supervision so that patients who present with complex needs are referred/escalated via the appropriate care pathway.

6. Conduct patient encounters with sensitivity and to act as patient advocate, maintaining patient confidentiality.

7. Deliver care that is patient centred, respecting diverse cultural backgrounds, and requirements.

8. Work in partnership with the patients to empower them to make informed choices about their healthcare and support choices.
