We are seeking an experienced Triage Nurse to join our Clients team and manage the care of 6,500 patients via phone. This is a temporary role for 6-8 weeks.Location: BracknellContract Duration: 6-8 weeksWorking Hours: Monday to Friday, 08:00 - 18:00Salary: £21.97 per hour (PAYE)Key Responsibilities:Patient Management: Handle triage for 6,500 patients using a traffic light system. Nurses will focus on "red" patients who require immediate attention.Questionnaire Review: Monitor weekly and daily patient questionnaires to assess their health status, ensuring timely and accurate reportingStatus Monitoring: Utilize advanced monitoring software to track patient conditions and identify those in need of urgent care.Immediate Response: Contact "red" patients to provide immediate support and make necessary referrals, including to General Practitioners (GP), Urgent Community Response (UCR) teams, or emergency services as required.Requirements:Registered Nurse with valid NMC registrationExperience in triage or similar rolesExcellent communication and organizational skillsAbility to work independently and manage a large patient cohortProficient in using monitoring and reporting softwareFor more information, feel free to contact Jake on (url removed) or call him on (phone number removed).We look forward to working with you
