Main duties of the job
This role focuses on working collaboratively across the Integrated CareBoard (ICB) and with external partners to understand and address localhealthcare needs and establish best practices aligned with Somersets healthpriorities. Key responsibilities include leading pathway development to createa whole-system approach and overseeing designated programs of work. This rolerequires championing NHS Somerset's values and promoting equality, diversity,transparency, and innovation.
Acting as a deputy for the Associate Director of Neighbourhoods whennecessary, this role works with local partners to identify and implementpriorities consistent with national requirements and helps develop strategicand business objectives for both the ICB and the Integrated Care System (ICS).Managing commissioning processes within financial and policy constraints tomeet national and ICB priorities set through annual planning is crucial.
The role includes project management of various programs, supporting thedevelopment, monitoring, and evaluation of plans, and creating reports andpresentations. It involves building collaborative relationships withstakeholders to maintain engagement and using communication, negotiation, andinfluence to commission services effectively. This entails setting KeyPerformance Indicators, monitoring quality and safety standards, particularlyfor mental health services, and collaborating with contracting and qualityteams to address issues as they arise.

PURPOSE OF THE ROLE:
The Transformation Manager of Neighbourhood Development will work closely with the Associate Director of Neighbourhood Development, directors and senior managers for their area (internally and externally) to ensure oversight and delivery of joint responsibilities. They will lead in the areas of:
Supporting the development and the delivery of the strategy for Neighbourhoods including Community Services, Personalised Care, Long Term Conditions, End of Life Care, VCFSE and Ageing Well, that in turn delivers the outcomes required for the people of Somerset when accessing care.
Support and lead on the delivery of the transformation agenda for pathways of care within Neighbourhoods.
The postholder will support the Associate Director in developing and delivering the commissioning, service re-design and transformation in all areas of Neighbourhoods, working collaboratively with system partners. The Transformation Manager will be responsible for ensuring that transformation delivery plans are developed, maintained and delivered, in line with organisational governance arrangements, to support effective service redesign, commissioning and performance management in partnership with system colleagues.
The Transformation Manager will also support the Associate Directors within the Strategic Commissioning team in work relating to their assigned locality, specifically around place-based commissioning at a neighbourhood level.
The Transformation Manager will support collaborative working across organisational boundaries by building strong and trusting relationships with key system partners to improve outcomes for the population of Somerset.
KEY RESPONSIBILTIES OF THE ROLE:
Working with all directorates within the ICB and with partners across the system, ensuring a good understanding of local need and best practice in respect of all commissioned services in line with agreed priorities for Somerset.
