Job summary

An exciting opportunity has arisen to join the LCHS Community Parkinson's service as a Parkinson's Disease Nurse Specialist.

Parkinson's Nurses have specialist experience, knowledge and skills and play a vital role in providing expert care to people with Parkinson's/Atypical Parkinsonism and their families/carers. The post holder will act as an expert resource for other healthcare professionals. They will provide expert advice and information to people with Parkinson's in order to facilitate and support self management of their condition.

The service offers a range of consultation options for patients including telephone, video and face to face at patients home, care home or clinic settings. The service also supports patients in a virtual ward model which offers enhanced and digital monitoring for more complex symptom management. The team in reach into the community hospitals, and work jointly with the Parkinson's team within the acute trust. The team triage contacts coming into the service to ensure individualised patient centred care.

The service supports people in all stages of the condition from diagnosis to the palliative stages of Parkinson's which enables joint working with a variety of health care professionals.

The team supports patients countywide in Lincolnshire so access to transport is essential.

Main duties of the job

The post holder will be responsible for delivering high level personalised care to those people with Parkinson's and Atypical Parkinsonism in a variety of settings and locations, as well as delivering educational training sessions to the wider multi-disciplinary team to support the people in Lincolnshire living with the condition.

Job description

Job responsibilities

Maintain responsibility and accountability for the provision of a high quality cost effective Parkinsons service based on health assessments of patients referred to the with the skill-mixed team to develop individual, evidence-based holistic care plans with the patient, carers, relatives and health and social care professionals.

Manages a caseload of patients with a diagnosis of Parkinsons Disease referred by GPs, secondary care and various agencies, establishing effective working relationships with patients, their families and carers. This will include education and promoting individual rights and recognising and respecting their contributions to care planning and delivery.

Establish individuals functional capabilities with regard to clinical management of Parkinsons Disease Using high level communication skills to communicate complex issues with patients and carers, in particular the skills needed for cognitive assessment and mental health status.

Maintain and record accurate documentation and correspondence in line with professional and trust guidance regarding nurse prescribing, interactions between clients, colleagues and other agencies in the appropriate legal records and formats. Utilising variety of technology platforms including Care Portal, SystmOne and Mobile Working.

Work collaboratively with colleagues and use data bases to actively seek out patients who will benefit from case management. Plan, implement, monitor and review interventions with individuals/carers through holistic patient centred assessments
