Job summary

The Rutland Health Primary Care Network (PCN) Clinical pharmacist will work as part of a multi-disciplinary team and have a key role in supporting the PCN to deliver the Network Contract DES specifications. You will complement and support the existing team of pharmacists - introducing new ideas, network working, and developing clinical special interests. You will be working within your set practice of Uppingham Surgery, supporting the surgery to deliver highly effective, safe patient care.

For the Structured Medications Review and Optimisation requirements this will include tackling over-medication of patients, including inappropriate use of antibiotics, withdrawing medicines no longer needed through NHS-led programmes such as low priority prescribing, and medicines optimisation more widely. The post holder will be required to work enhanced access hours of 8pm one day per week. You will have access to CPPE and IP training if you do not already hold these qualifications.

The post holder will help ensure that the PCN practices integrate with community and hospital pharmacy to help utilise skill mix, improve patient outcomes, ensure better access to healthcare and help manage workload. The role is pivotal to improving the quality of care and operational efficiencies so requires motivation and passion to deliver excellent service within general practice.

Main duties of the job

Job responsibilities:

Patient Facing Clinical Services& Care

Work in a patient facing role to clinically assess and treat patientsusing their expert knowledge of medicines for specific disease areas Take responsibility for the care management of patients with chronicdiseases and undertake clinical medication reviews to proactively manage peoplewith complex polypharmacy, especially the elderly, people in care homes, thosewith multiple long term conditions (in particular COPD and asthma) and peoplewith learning disabilities or autism (through STOMP - Stopping Over MedicationProgramme) Conduct face to face and othermodes of medication reviews and agree medicines care plans in agreement with patients and carers Deliver long term conditions clinics and home visits particularlyfor patients with complicated medication regimes and prescribe accordingly Prescribe acute and repeat medication for patient need within areas of competency Provide targeted support and pro-active review for vulnerable, complex patients and those at risk of admission and re-admission to secondary care Provide specific advice and support to patients in care homes and support staff Provide help and advice on medicines use to patients and their carers

Medicines Optimisation Systems

Provide leadership on person centred medicines optimisation. Throughstructured medication reviews, support patients to take their medications toget the best from them, reduce waste and promote self-care Oversee system for reconciliationof medicines from outpatient and discharge letters and support safe transfer of care through liaison with hospital and community colleagues Liaise with other stakeholdersin the supply chain such as community pharmacists and appliance contractors to handle queries and manage supply problems Take a central role in the clinical aspects of shared care protocols,clinical research with medicines, liaison with specialist pharmacists(including mental health and reduction of inappropriate antipsychotic use inpeople with learning difficulties) and anticoagulation

Safety

Report medicines related incidents, contribute to investigations and rootcause analyses Participate in serious incident investigations and multidisciplinary casereviews Report Adverse Drug Reactions and promote reporting by MDT Action and respond to Drug Safety Alerts and other safety cascades.

Training, Education &Information

Develop and deliver education sessions and training to clinical and non-clinical staff on medicines issues Deliver education and support to individual patients and patient support groups on medicines Provide ad-hoc medicines advice to doctors and other clinicians Assess emergent clinical evidence and develop strategies for implementation in practice Horizon scan for new drugs and provide information to the MDT on their introduction Provide advice on cost effective prescribing and supply issues Provide teaching and support to medical students, GP trainees and other trainee healthcare professionals Actively participate in National CPPE training scheme for clinical pharmacists

Qualifications

Essential

A 4 year Master of Pharmacy degree approved by the General Pharmaceutical Council Membership as practising pharmacist with The General Pharmaceutical Council. Commitment towards continuing professional development in accordance with requirements of The General Pharmaceutical Council.

Desirable

Pharmacist independent prescriber registered with The General Pharmaceutical Council (preferred) or training to become an independent prescriber. CPPE pathway.
