Job summary

An exciting opportunity to work within our renal unit for a part time Band 6 registered nurse as an Advanced Kidney Care Nurse. The renal unit looks after our local population of renal patients as well as patients from further afield. The service cares for patients with kidney disease encompassing acute kidney injury, chronic kidney disease and renal transplantation. There are 2 haemodialysis units, one of which looks after acutely unwell patients and a home dialysis unit. Our outpatient clinics provide care for patients along their renal pathway. You will contribute to the provision of excellent patient care and must have experience and up to date knowledge and skills in renal nursing. You are expected to have excellent communication skills in order to liaise appropriately with key members of the multi-disciplinary team and both primary and secondary teams. You should be able to draw on extensive knowledge and skills and be comfortable in giving information to patients to help them make informed choices, have developed clinical decision make skills, be a confident educator for both patients and staff and be able to have a flexible approach in meeting the needs of patients. Opportunities for study, teaching, research and service development are available and encouraged as part of this role within the team. Applicants should be able to demonstrate excellent communication and team work skills.

Main duties of the job

Main Duties:

To work closely with the Consultant Nephrologists and AKC Lead to facilitate the management of Advanced Kidney Care patients encompassing anaemia management of the Chronic Kidney Disease population. To co-manage out-patient clinics at St Georges and lead clinics at Kingston Hospital on Monday and Friday afternoons with Consultant support. To deliver high quality, evidence based care to patients and their families. To act as a source of specialist nursing knowledge for the clinical staff. Independent and supplementary prescription within protocols and cost controls (if a nurse prescriber) within the guidelines set out by the Royal Pharmaceutical Society for prescribers. Review of prescription, make recommendations and implement change as necessary (if a nurse prescriber) within the guidelines set out by the Royal Pharmaceutical Society for prescribers. To undertake patient/staff education. To partake in the audit process of Advanced Kidney Care and anaemia management. To communicate with expertise within the multi-disciplinary team and with primary care providers. To facilitate learners and act as an effective role model. Maintain and develop own clinical expertise and credibility by undertaking clinical shifts as require.
