Job summary

We are looking for highly skilled outstanding Nurse Practitioners / Care Practitioners to join the Same Day Emergency Care Unit (SDEC) and Urgent Treatment Centre (UTC) on a rotational basis at the Friarage Hospital.

Our excellent team of nurse practitioners/ care practitioners provide clinical assessment, treatment and management of acute medical problems and immediate care for a range of minor injuries and illnesses for the population of Northallerton and surrounding areas in North Yorkshire.

SDEC and UTC are friendly, autonomous and forward-thinking departments within the Friarage Hospital which is part of the Growing the Friarage and Community collaborative and the wider South Tees Hospitals NHS Foundation Trust. They deliver high quality care to a rural population of approximately ,141,000 people serving an area of 1,000 square miles.

South Tees Hospitals is the largest hospital trust in the Tees Valley with two acute hospitals and services in a number of community hospitals.

The Friarage Hospital in Northallerton is an essential part of our organisation and an important point of delivery for the community.

The services have links with all specialities including the Hospital at Home Team, Urgent Community Response Team, Mental Health, Paediatrics etc .

Main duties of the job

The Nurse Practitioner/ Care Practitioner will continuously evaluate their role supporting patients with complex and urgent care needs. They will work with the area professional leads and in partnership with local consultants, GP practices and the local PCN's to further develop services which are of value to the commissioners and meet the local needs of the population. We work collaboratively with primary care and acute care consultants to prioritise in avoiding unnecessary admissions to hospital and inappropriate attendances to ED.

The post holder will work autonomously as part of the SDEC and UTC team to provide an assessment, diagnosis and undertake treatments/interventions and care for patients with differential diagnosis by following agreed pathways.

Whilst on rotation with the SDEC team the successful candidates will provide the hospital at night out of hours service, giving support to the junior medical team and the hospital site with advanced clinical skills.
