Job summary

We arerecruiting a programme coordinator to support people living with a palliativecare or end of life diagnosis across Birmingham to maximise their quality oflife at home.

Main duties of the job

The Programme Coordinator will work as a team member and be responsible for taking referrals, visiting clients, assessing needs, effectively organising and coordinating services/support, navigating clients to other appropriate support/services, advocating on their behalf or accompanying them as required, supporting their end of life/palliative care journey.

Job description

Job responsibilities

To work with clients and their families enablingthem to access appropriate support services to maximise their quality of lifeat home.

To assess clients needs using the Bridgesassessment procedures. Visiting and meeting clients in their homes, hospital,hospice or other appropriate setting as preferred by client.

To carry out a narrative based assessment andnetwork mapping exercise to understand clients needs and understand what isimportant to them. The network mapping exercise will enable the ProgrammeCoordinator to help the client to understand their care and support networksand how this support can be best utilised.

To set up appropriate support as identified duringthe assessment; this will vary from client to client. The Programme Coordinatorwill need to coordinate support, sourcing additional support where needed,which could be provided from the approved list of providers. The ProgrammeCoordinator will ensure clients are able to access all appropriate services benefit advice.

When required, support clients in accompanying andadvocating on their behalf with different service providers and agencies.

To record accurate data for service monitoring andevaluation including recording accurate up to date comprehensive notes onclient files, working within the client information handling guidelines.

Record new referrals and client interventions ontoexisting database and maintain both the database and other records accurately.

To provide regular reports as required, includingend of quarter and end of year reporting.

To action agreed support and raise purchase ordersforms in compliance to MHCT procedures working within and monitoring the budgetspend.

Good standard general education and qualification in basic skills.

Experience of using IT software word, excel. Experience of working with members of the public in a health and social care environment. Experience of working with individuals and groups in a domestic/home environment. Experience of working with multi-disciplinary teams. Experience of working as an effective team player.

Desirable

Experience of working directly with end of life/palliative care service users. Experience of working with bereavement. Experience of outreach work.
