Job summary
The post holder is responsible for the assessment, planning, implementation, and evaluation of evidenced-based nursingcare, working collaboratively and co-operatively with others to meet the needs of patients and their in the management and organisation of their clinical/specialist area participate in the education, development, andsupervision of other staff Assuming all reasonable precautions for a safe and secure environment for self and others in accordance with Healthand Safety legislation reporting any areas of concern to the Nurse in Chargeo Promote excellence and improve standards of nursing care by being involved in audit within the clinical area control auditso Participate in the monitoring of standards and quality of nursing care, through benchmarking, audit, and Participate in patient and public involvement activities, promote people's equality, diversity, and rights.
Main duties of the job
To support the Team in ensuring the efficiency of the screening and monitoring process. Monitor and participate in the process to ensure that patients are physically well enough to proceed with treatment, and request further medical intervention as appropriate.
To provide effective communication with patients and their families in the plan of care, including initiation and cessation of medication or other interventions,
Undertake nursing assessments of patients referred to the Service and ensure effective communication with multidisciplinary team.
Ensure patients/relatives/carers are provided with relevant information regarding their diagnosis/illness and providing referral or signposting to appropriate services as required.
If necessary attend / chair Decision Support Tool meetings providing relevant documentation when indicated.
To undertake, risk assessments as necessary and plan care to meet patient needs, in collaboration with the patient and their family ensure implementation of agreed care plan.
Effectively utilise specialist knowledge and skills to assess the changing needs of patients and effects of medication.
To work autonomously in the community setting under the professional guidance of the Team Manager, following individual patient care plans.
Have knowledge of the legal frameworks such as Mental Health Act legislation, Mental Capacity Act and Deprivation of Liberty , the Mental Health Measures (Wales) and Health and Social Care Wellbeing Act
