Job Overview

We are seeking a skilled Mental Health Care Coordinator to join our multidisciplinary team at Swansea Bay University Health Board. As a key member of the Community Mental Health Team (CMHT), you will be responsible for assessing, implementing, and evaluating care and treatment plans for individuals on your caseload.

Main Responsibilities

You will work autonomously but under the indirect supervision of a Band 7 registered nurse/team manager and in conjunction with other health colleagues to deliver a high-quality community mental health service.

Key duties include:

  • ">
  • Assessing and developing individualized care plans in collaboration with service users, carers, and healthcare professionals;">
  • Implementing and evaluating agreed-upon care and treatment plans to ensure optimal outcomes for service users;">
  • Maintaining accurate and up-to-date records of patient progress and interventions;">
  • Collaborating with multidisciplinary teams to coordinate care and support services;">
  • Providing education and guidance to service users, carers, and healthcare professionals on mental health issues and related topics.">

Requirements and Qualifications

">

To be successful in this role, you must possess:

  • ">
  • A Registered Mental Health Nurse qualification;">
  • Evidence of post-registration study, such as a postgraduate diploma or equivalent registration experience;">
  • Demonstrated clinical knowledge of mental health legislation, including the Mental Health Act 1983 and the Mental Capacity Act;">
  • An understanding of recovery-oriented care principles and practices;">
  • Excellent communication and interpersonal skills, with the ability to work effectively with diverse groups, including service users, carers, and healthcare professionals.">

Desirable Skills and Qualifications

Prior experience working in mental health services, leadership roles, or advanced practice positions is highly desirable.
