Job summary

Lead ReferralsOut Administrator is a new position we areable to offer as part of DHC expansion.We have seen an increase in our patient activityrecently and we would like to expand the team so that we can continue toprovide the excellent standard of care expected of us.

The candidate will be an integral part of Outpatients, overseeing the Referrals out team whoprocess patients paperwork who have been referred out for diagnostics orprocedures, ensuring that the process is both efficient and effective. This is achieved by accurately processing andactively monitoring the flow of information throughout the patientpathway.

Main duties of the job

Organisation, prioritisation, IT skills and People Management. Attention to detail will be crucial in thisrole as you will be overseeing and creating referrals on patient records toother providers. You will need to gatherall the relevant information from the patient record system, submit it viaemail to the provider & update the patient record.

Fulltraining will be given on the patient record system and how to complete therole.

DHC is a GP Federation, representing 140,000 patients providing a wide range of quality healthcare services including elective care, Talking Therapies, community healthcare and primary care across Surrey. We are registered with the CQC (Care Quality Commission) and we are an accredited AQP (Any Qualified Provider).

DHC is working at the centre of the local health system to evolve and deliver integrated care and that requires strong partnerships with many organisations. In all of our services, we aim to put the patient at the centre of their care, make sure our services are easy to access by local patients and reduce waiting times for patients.

GP practices have been brought together into Primary Care Networks (PCNs) to offer services in the local community. There are 4 Primary Care Networks as part of the DHC Federation, Redhill Phoenix PCN, Dorking PCN, South Tandridge PCN and Integrated Care Partnership PCN. The PCNs are critical to the changing NHS and are the focal point of all locally developed planning and service delivery.

Job responsibilities

The role is to lead the Referrals-Out team who areresponsible for an integral part of the patient pathway. Coordinating the teamon a daily basis ensuring they undertake tasks essential to the smooth andeffective provision of quality care; providing an efficient level of servicefor patients and their families.

The role will ensure the team works within thepriorities and expectations set out by the Outpatient Management team;regularly reporting team progress and output.

It is essential the post holder exercises peoplemanagements skills, initiative and judgement whilst maintaining a high level ofconfidentiality and diplomacy at all times.
