Job summary

This is an exciting opportunity for a full time band 6 physiotherapist to work in a newly created post specialising in palliative care at Forest Holme Hospice based within the University Hospitals Dorset Trust.

We are looking for a physiotherapist with a passion for working in palliative care who would like to join the specialist MDT at Forest Holme Hospice in Poole. Situated in a beautiful part of the country close to the Jurassic coastline, lovely beaches and New Forest National Park, the hospice provides specialist palliative care to those with life limiting illness in a supportive and caring environment.

The post holder will deliver high levels of evidenced based therapy intervention, implement appropriate protocols and national guidelines, facilitate training and supervision for the MDT and junior therapy staff and lead on service improvement. You will have previous experience working in palliative care and be passionate about providing individualised care for patients with complex needs.

Professional support and mentorship will be provided by the therapy team lead at Macmillan Hospice with opportunity to access IST and CPD to enhance professional skills and knowledge.

This is a full time permanent post with hours worked Monday - Friday. Currently there is no on call, shift, evening or weekend working attached to this role. Job share considered subject to appropriate needs of the service being met.

Main duties of the job

The post holder will provide a high standard of therapeutic Intervention to an 11 bedded inpatient hospice. As a senior member of the multidisciplinary team, you will provide evidence based individualised care to achieve optimal levels of functional performance, facilitate preferred place of care and optimise symptom control to maximise quality of life and address 'what matters' for each person.

The post involves some 'outreach' into the community to provide specialist assessment to inform on appropriate hospice admission or facilitate complex discharge planning. As such, you will need to be confidant working as an autonomous practitioner with some community professional experience an advantage.

You will takean active role in quality improvement and service development, and provide senior support and supervision to junior team members and the multi-disciplinary team to both enhance quality of service and identify / minimise risk.

The successful applicant will need to have strong communications skills, be highly organised, good at working closely within a multi professional team and have knowledge of external agencies appropriate to the clinical caseload in order to coordinate appropriate treatment and care.

Degree / Diploma in Physiotherapy HCPC registration

Desirable

Member of the Chartered Society of Physiotherapy Advanced communication training Post graduate training in palliative care / oncology QI / Leadership / Service improvement post grad training

Experience

Band 5 experience of all core rotations Experience working with patients with a palliative diagnosis Experience of supervision of students / junior colleagues

Desirable

Experience working in a band 6 role Clinical Experience working in a community setting Experience of Advanced Care planning and it's use in clinical practice Experience of delivering supervision and appraisal to junior colleagues

Technical Skills / Competencies
