Job summary

An exciting prospect has arisen for a Specialist Community Nursery Nurse (SCNN) to work within the Specialist Nursing Service at the Child Development Centre.

The successful candidate will work in partnership with families and professionals, delivering both clinical care tasks and behaviour, sleep, continence support strategies in order to make a positive difference to individuals and families.

A key part of the role will be facilitating both individual and group sessions both on TEAMS and in person. In-depth training, clinical support and induction will be offered before you work independently.

You will work with children, young people and families and delivering care in clinic, educational and other community settings as the need arises.

The successful candidate needs to be able to communicate and organise effectively, prioritise tasks, be methodical in their thinking and continually exercise their initiative.

Main duties of the job

To work within the Community Paediatrics team supporting the clinical assessment of SEND at the earliest opportunity; using your clinical skills and knowledge, you will follow up with families in the home, schools or other settings.

To work directly with families identified as needing support. Appropriate referral to be made where a medical or health need has been identified. This would include providing support and clinical advice to parents.

To take an active role in workshops with parents (online and in person) providing advice and support.

To assess the appropriate support services required and to facilitate access, which will include supporting the onward referral to support services or for clinical interventions.

To work with all professionals to identify early support needs with accessing early education settings.

To guide families to appropriate Universal support.

Participate in Multi-Disciplinary Assessments to act as a liaison between professionals and parents of child.

To liaise with schools and nursery settings to ensure a needs led approach is provided for children and young people where it is need.

To act as a chaperone for child protection medicals as required.

To take part in service development projects as required.

To deliver QB testing on a regular basis.

To take blood pressure and pulse readings, and complete growth monitoring for children and young people. Training will be available for this.

To ensure a safe environment and the safe use of equipment and reporting any unsafe environment or unsafe use of equipment to the Team Leader. To identify changes in the childs condition assessing and referring appropriately to parents and/ or community nurse. In an emergency know what to do and who to contact. Communication

Clinical and Practice Governance

Observe and maintain strict confidentiality with regards to any patient/family/staff/records and information in line with the requirements of the Data Protection Act. Any data that is taken/shared as part of a phone call or transported, faxed, or transferred electronically must be undertaken with regard to the Trust Information Governance and Information Security policies. The post holder must adhere to the Trust risk assessment and risk management processes The post holder must adhere to infection control policies and procedures Undertake mandatory training and any other training relevant to the role as required by Cambridgeshire Community Services NHS Trust.
