We specialize in complex care such as nursing care, PEG Feeding Management, tracheostomy care, spinal injury care, and more. Our staff are highly skilled and trained to provide nursing and home care services, including support for dementia, diabetes, cerebral palsy, and other disabilities.


Role Description

This is a full-time on-site role for a Care Coordinator at Verity Healthcare in Cornwall. The Care Coordinator will be responsible for care coordination, case management, communication with clients and families, providing home care services, and delivering exceptional customer service on a daily basis.


Qualifications

  • Care Coordination and Case Management skills
  • Strong communication skills
  • Experience in Home Care and Customer Service
  • Compassionate and empathetic towards individuals of all ages
  • Ability to work well in a team and independently
  • Knowledge of healthcare regulations and practices
  • Certification or degree in a related field (NVQ Level 2 or 3)


Requirements

  • Full UK driver's license.
