**Role Snapshot**
To ensure a swift co-ordinated accommodation plan is in place for high need clients who are homeless and/or rough sleeping, to access suitable accommodation and services to improve outcomes and prevent the reoccurrence of homelessness.
**What you could be part of**
You will be part of the small Rough Sleeper Team working to prevent homelessness and ensure that no one has to sleep on the streets.
**What you’ll be doing**
- Complete assessments for each customers support needs and risk to allocate a suitable placement within the supported accommodation provision where appropriate.
- Create robust working relationships with existing residents in supported accommodation to allow quick move on to long term accommodation.
- Foster and develop proactive working with supported accommodation providers to identify customers move on options.
- Work proactively with agencies to mitigate and reduce risk, build positive working relationships with both customer and external agencies to source and signpost to support.
- Work with agencies and support providers to move on customers to more permanent housing in the private and social sectors.
- Investigate any complaints by customers and landlords to resolve issues as they arise.
- Take direction from the Senior Rough Sleeper Coordinator to achieve positive outcomes for rough sleepers accessing the service.
**You must have**
- 5 GCSEs or equivalent (including Maths and English) or the ability to demonstrate ability at this level.
- Full UK driving license and access to a vehicle.
- A proven understanding of housing management, managing rent accounts, tenancy administration, and completing risk assessments and support plans.
- Significant experience of working with clients who need a high level of support and who have complex needs and experience of building relationships based on collaboration and trust with accommodation and support providers.
- Experience of working with appropriate partners to identify appropriate solutions for clients with thorough knowledge of local services landscape, including Mental Health, Health, Welfare and Benefits etc.
