Practice Plus Group are looking for a Patient and Stakeholder Engagement Lead to develop and deliver an Integrated Urgent Care engagement strategy across our **London contracts: 111, Out of Hours and Clinical Assessment Service**.

You will be working closely with **patients, stakeholders, strategic partners and providers** to integrate new resources into our Clinical Hubs. You will be involved in local level, London system and national meetings to help in development and implementation of innovation and change. This role will evolve over time to include new **Service Developments **in social care, community health, palliative, paediatric and geriatric care.

You will be responsible for overseeing the development of **excellent patient pathways** and ensure services are accurately reflected. Keeping the Directory of Service up to date you will guarantee the IUC Service is able to deliver against national and local objectives.

This is a key local, mid-senior role working in tandem with colleagues to develop the IUC strategy, innovation and service improvements for National IUC models.

**This role will be based in our Putney office with some home working. **Relocation packages can be offered if needed.

**About You**

You will be able to build and maintain strong working relationships with stakeholders at all levels and possess a positive and professional attitude with a high level of work organisation, self-motivation, drive for performance and improvement and flexibility in approach and attitude.
- Clinical experience/knowledge (Advanced Clinical Practitioner) - **but not essential**
- Excellent interpersonal skills.
- Demonstrated Initiative skills
- A strong sense of personal and team accountability
- Highly effective verbal and written skills
- The ability to write reports and other business correspondence.
